Posted on Thu, 12 Feb 2015
Question: Iam from the Philippines and on vacation right now in Australia. My Doctor prescribed me with harnal ocas 400 mcg twice daily once in the morning and once in the evening. Is it safe? I've read that it may cause hypotension. I also want to know if the medicine is available in Perth. I've been taking harnal ocas 400 mcg once a day for 7 years. I also want to know what other options available.
How can i get the apps?

Hello

Welcome to Health Care Magic.
My name is Dr Muhammad Ahmad & i will look into your problem.

I have gone through your query and would try to help you in the best possible way.

I understand your concern and will answer your questions one by one:

1- Is it safe:

Yes it has quite a good safety record .

2- Hypotension:

Yes it causes hypotension , not in every one but in very few people , if you have been taking Harnal Ocas already, and did not suffer any hypotension, In my opinion you have very low chances of suffering any such issues with this.

It may also cause eye problems in a very few number of patients so keep a close look at your eye if you feel any issue ever consult your doctor.

3- Available in Perth

Yes it is available in Perth with the name of Flomax 0.4 Mg or you can just ask the chemist for Tamsulosin Hydrochloride 0.4 mg preparation.

OTHER OPTIONS:

For this i would like to know about your reports PSA, findings , biopsy findings if any, PSA levels and diagnosis and symptoms if any, You can upload them here with attach report option.

How can i get the apps?

I didn't get what do you mean by "apps" please explain.

My symptoms are urinary retention and hesitancy and I get up at least 3 times during nightime just to pee although small amount of urine only. About a week ago, the symptoms get worse and I felt like my medication does not help anymore. I felt pain on the genital, bladder area, lower back and felt pressure on the kidney area. There were times that I'm afraid I might have an acute urinary retention. In order to extract more urine from my bladder I'm using a plastic cup to catch the urine from my genital and the same time massaging the under portion of my genital against the plastic cup just to squeeze up urine. Sounds weird but I have to do all means in order to void. What other advice can you give me to help me improve my symptoms? I was diagnosed with parkinson's disease last March 2013 and was told that it's part of parkindons symptoms.

Brief Answer:
There are some options.

Detailed Answer:
Hi,

Thanks for writing back in such detail,

I am sorry to hear that this problem is causing you so much difficulty to deal with. But i can assure you that with proper treatment things can get way better.

Your symptoms point towards Benign prostatic hyperplasia... there are a number of options which we have if medication does not work, if your doctor has prescribed you harnal ocas i will ask you to keep using it and see if thy symptoms subside.

If symptoms do not subside and things stay same or worsen ... there is always option of Trans-urethral resection of the prostate (TURP) which is a very safe procedure with great result and without any cut.

You can discuss this option with your doctor as well.

Prostate issues are not really part of Parkinson's disease but sometimes bladder and urination issues can be a part of Parkinson's disease. Your doctor suggested you prostate med it means you have prostate issue, neurological issues because of Parkinson may be there .

SUGGESTIONS:

Meet a good urologist if meds don't work and discuss about TURP and other treatment options.

Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!
