Question: I had a cesearean delivery on 17.11.2014..because of stomach pain inl was prescribed abdominal mri on 12.01.14..now a repetead mri prescribed again.is repeated mri safe for me again post delivery.does an mri shows adhesions, perinotistis incisional hernia or distended abdominal wall

Yes, repeat MRI is necessary in your case. It is better if you gone through it.

MRI is non hazardous and you can go for it. It uses magnetic field to detect the abnormality. Magnetic field has no effect on the body. It is only contraindicated if you have heart valve replacement, pace maker or cochlear implant.

In contrary to it X-ray uses ionizing radiation and can affect female gonads e.g. ovary. So frequent exposure of X rays are avoided but you can go for MRI for sure.

Yes, it can detect adhesions, peritonitis, incisional hernia or distended abdominal wall with some limitation. But it will diagnose up to extent which can help you.

Your MRI report are clear except some fluid collection. You have taken lots of antibiotics so it is unlikely to be infection. This fluid collection may be as a reaction to surgery. So second MRI or CT can help establish the exact nature of fluid collection.

You should also do stool for culture and sensitivity. It will guide you regarding the possible bacterial infection, which I think is highly unlikely in your case.

Please avoid constipation by eating high fiber diet like green leafy vegetables etc. Pain due to constipation can be cleared up this way.

Two months have elapsed after the surgery so I advice you to start the exercise. Abdominal, perineal and leg exercise can help you clear rest of your symptoms. You can also visit the physiotherapist they will guide you regarding how to do exercise.

Exercise wants some time to start taking its effect once the effect started it will built up gradually and you will be benefited with the symptoms.

Thanks doc.will be going for mri tommorow but I m having pain in my rib cage on right side as well as numbness in my hands and head..will it be safe for me to go for mri..and as per your findings of mri report if there is some fluid collection y is it not subsiding post so much of antibiotic treatment. .is there a possibility of lesions or adhesions in that area..what would be

MRI has no effect on your numbness, you can surely go for it. It will tell us your recent status of fluid and any other related changes.

It is not necessary that fluid always collects because of infection. Some fluid collection is of reactive in nature. It arises because of the surgery and wanes off gradually with time.

Yes, its possible that fluid may arise because of the adhesion. If Its adhesion then it will be seen in the MRI reports and surgeon can later correct it operatively.

No, It wont be endometriosis because in endometriosis you also face the symptoms of excruciating abdominal pain associated with menstrual period. Changes of possible endometriosis can also be known in MRI report.

Yes, MRI finding can be obscured with continued menstruation bleeding. This is because of bleeding and hypertrophied endometrium.

Your MRI report is not so urgent so we can wait for few days. So in my advice you can do your MRI report after completion of your period.

which is the best mri 1.5 or 3.0 sir?? whats the difference..i dnt wanna go for a contrasts study as well as ct...but will mri be helpful for me if there is some nerve compression or something else..will mri b able to detect that

3.0 T is better than 1.5 T. 3.0 T has good signal intensity and short exposure time. So if you have option of 3.0 T, than you should select it.

CT with contrast has some risk of radiation exposure, but it will give you good overall view of abdomen than MRI. But doing CT for once is not a risky and you should select it.

MRI is more specific in nerve compression than CT.

So, both investigation has its pros and cons. You should select it according to your preference and doctors guidance.

Thanks for the query.

If there are lesions of suspicious tuberculosis in the properitoneal area than it should be confirmed by microbiologically.

I advice you to carry out ESR testing along with stool for AFB testing.

If the above report is showing evidence of infection than you can test the TB bacilli for the GeneXpert testing. It will give you a idea regarding primary resistance of rifampicin in TB bacilli. This will be help ful in the treatment.

I have gone through your hemogram report which is stating that your ESR is somewhat higher. In case of tuberculosis it is higher (much much higher than here).

So, there is very little chance of tuberculosis to you. But radiology is directing us towards the TB.

So better is to rule out the tb from you.

I advice you should go for ZN staining from the stool. This will stain and diagnose any tuberculosis bacteria found in the stool. This will diagnose the TB infection only.

GeneXpert can diagnose as well as detect the resistance of rifampicin drug. This is PCR type method and very sensitive for detection of the tuberculosis. This will solve all your mind queries. It is very handy investigation and can give result in few hours.

There is no need to do MRI now. Our prime target now is to rule out the TB from you, which from your body habitus I think is unlikely. But confirmation should be done.
