Posted on Fri, 16 Jan 2015
Question: I had my first infusion of reclast today (no discussion, XXXXXX simply presented is as a drug instead of Pamidronate which I have been on for over a year (need calcium as stage 4 mesastasis of breast cancer treatment has caused bone damage) - My CA15 has gone from 1100 to 37 (today) - I have light side effects on Kadcyla and Pamidronate, but after reading about Reclast I am concerned that this continues. I am seeing my oncologist tomorrow. What should I ask her? (I am surprised she did not discuss this change with me). thanks

Brief answer: Zoledronic Acid is superior to Pamidronate in most cases...

Detailed answer:

Hello,

Thanks for the query.
I have gone through your query and understood your concerns.

Both ZOLEDRONIC ACID (Reclast) and PALMIDRONATE belong to a class of drugs known as BISPHOSPHONATES.

Clinical trials suggest that RECLAST ( ZOLEDRONIC ACID ) is definitely superior to PAMIDRONATE. Better survival and bone density results.

Patients with breast cancer suffer significant amount of skeletal morbidity during the natural course of the disease.

Bone metastasis occur in 65-75 % patients of advanced breast cancer.Such patients tend to have 3-4 skeletal events in an year and in general their first fracture tend to occur within 7-9 months of the diagnosis of bone metastasis.

ZOLEDRONIC ACID ( RECLAST ) has shown favorable effects and safety for the treatment of bony lesions of breast cancer.

Studies show that a 15 minute infusion of RECLAST has a positive Impact on patients quality of life.

In the end, I would say that decision of your doctor to give RECLAST instead of Pamidronate is perfectly alright. If you have side effects on Pamidronate.. then we need to find you something that suits you better.

You will need to give the drug some time to work and get to a steady state in your system, so don't worry too much about the Ca15 level just now.. there will be some tweaking involved with the dosages.
I hope it helps however you may revert to me for any further query.
