Brief Answer:
More evaluations is required.

Detailed Answer:
Hello,
Thanks for the query.
I have gone through the image and I can understand your concerns.
As per your description and appearance on image, it can be an abscess( pus collection) however as there is puckering of skin we have to rule out possibility of an underlying lump.
You have to get an ultrasound of breast at the earliest. If there is an underlying lump or mass additional investigations like fine needle biopsy may be required. If it is a simple collection, anntu and drainage of pis will be enough.
Hope I have answered your question.
Thanks
