Apologies for the late reply.

Long term use of vesicare is generally not harmful. In view of its antimuscarinic properties you may have developed oral dryness which can lead to bad oral hygeine; constipation; and palpitation. But beyond these no significant adverse effects are found.

However it would be even better if we addressed the cause if possible rather than using drugs to control symptoms. In a female of your age, the common cause are related to uterus and vaginal health. Therefore it is apt you see a gynecologist if not an urogynecologist first.
Though I don't have first hand knowledge of medical centres at Nottingham, from what I read Nottingham University Hospital is well equipped and is really nice institute. You can consider visiting gynecology centre there. I found this phone number matching on websearch results - 0115 924 9924 that connects to gynecology centre at NUH.
