Is Low TSH Level Causing Hypothyroidism Or Hyperthyroidism Symptoms?
Question: My tsh is 0.09. The normal range for their lab is 0.27-4.20. I have recently completed menopause and am 55 years old. I was diagnosed with Hashimoto's Hypothyroidism at age 25. I was given lab tests for this four times last year. The 0.09 result is from January. I am going back to the Lab tomorrow (they should have sent me sooner). The T4 is normal. Could these changes be due to menopause? Aging? Is this tsh level causing hypothyroidism or hyperthyroidism symptoms?

A low TSH is suggestive for hyperthyroidism or overacting thyroid.
If FT4 and Ft3 are within the normal ranges, than it is subclinical hyperthyroidism, manifesting usually with mild symptoms.
Very rare happens that Hashimoto converts to hyperthyroidism, or it may be a nodular goiter coexisting with Hashimoto disease that has caused hyperthyroidism. Some radiologic exams are needed (thyroid ultrasound, scintiscan).

The Ft3 lab test was not done, but the Free Thyroxine is 1.5 mg/dL. Thank you for letting me know that further tests are needed. I will be contacting an Endocrinologist at Massachusetts General.
The current dosage of Synthroid is .75 and they want to move it to .88. This has been tried in the past, unsuccessfully.

If you are under 75 mcg of Synthroid, than the low TSH is due to Synthroid treatment.
You need to lower and not to increase the dose as a low TSH is suggestive for over dosage.
I would suggest to continue with 62.5 mcg and check TSH after 6 weeks to be sure that the dose is the right one.
I still recommend a thyroid ultrasound if you haven' t done it for more than one year.
