I read your query and here is my advice.
I don't think the muscles twitching or cramps are much related to ALS.
If it was ALS it would have had other symptoms like muscle weakness, falls, inability to do normal every day activities, difficulty walking etc.
The symptoms of mussels twitching and cramps may be related to neuropathy and it can be with diabetes origin.
I can suggest you to do a fasting glucose levels, post prandial glucose levels and HbA1c to get tested for diabetes.
Also electrolytes imbalance and dehydration may cause it especially if you have been using diuretics.
Pregabalin may help with cramps. Also I can recommend you to take vitamin B 12 supplements.

I can recommend you also to test natrium, potassium and calcium levels in blood. Electrolyte imbalance can cause twitching.
