Brief Answer:
The weight loss is significant

Detailed Answer:
Hello and Welcome

I appreciate your concern

The BUN, creatinine, creatinine clearance and FBS are normal. The weight loss is significant. Greater than 10 % of normal body weight if lost, is considered as protein energy malnutrition. Complete blood counts, Chest x ray, ESR/CRP, sputum for AFB , hepatitis profile and Liver function tests should also be done to look for malignancies, tuberculosis and liver pathology. If you had a higher BMI and lost weight accordingly then these investigations may not be required especially if you are asymptomatic and having no features like fever, night sweats, lymphadenopathy. As you mentioned the weight loss was intentional then there doesn't seem to be a function of age related to this weight loss. One should not loose 45 pounds over 6 months normally at any age without an underlying cause unless a strict diet and exercise plan was followed.

Hello again.

Respected Madam.

I answered the question, keeping in mind that you intended to loose weight. You may ask another MD the same question but the workup will remain the same for such weight loss. I commented on the investigation findings and the due workup required for such a case.

The RFTs are normal.

You did not mention your BMI and current weight. You may also mention whether or not you have had any thyroid abnormalities in the past

You did not mention your active complaint as you stated that you were going to gym 5-6 times per week.

Kindly explain this sentence as its not quite clear what specifically its inquiring about.

"What is going on? this just a function of age? "

I am sorry if my answer was not satisfactory. You may ask for another doctor from the HCM team.

Have a great day and wishing you all the best.

Hello again.

No worries madam. Maybe I could not properly grasp what you were trying to convey. I hope you would accept my apology for that as I care deeply for my patients and consider them my ultimate family.

Both the creatinine and BUN are normal.

The normal range of creatinine for women is 0.6 to 1.1 mg/dL . Creatinine is the main measure of renal function.

The BUN can become elevated in all forms of renal failure and corresponds mainly to the severity or degree of renal failure. It can also be come elevated when renal function is normal e.g in response to a high protein diet or upper GI bleed.

If you are concerned , you can repeat the RFTs to see for a rise as 0.5 to 1 point rise is also significant for renal dysfunction.

If possible , kindly upload the images or a copy of the investigations that you sent
to assess them more accurately.
