Hello
Thanks for your query,based on the facts that you have posted it appears that you have been posted for Hydrocele surgery in near future and you have been cautioned about a need for removal of testis (Orchidectomy )if required.
Please note that in majority of cases of hydrocele surgery Orchidectomy is not required unless on exploration it turns out to be infected hydrocele.However this can be determined by doing Ultrasound Scanning of Scrotum before heading for surgery.Second option is to take antibiotics for couple weeks prior to surgery .
Since you have normal PSA .You have every right to refuse going for removal of testis( Orchidectomy) and give consent for hydrocele surgery only.
