Question: hello doctor,

one year back i have received a handjob from massage parlor after 3 months i got pain under my arm pits and its been comes and goes but now i see constant irritation and mild pain under my armpit.

I have taken hiv elisa test at two and half months after the exposure it came negative and i have taken several oraquick in home hiv test by 4,5,6 , 8 and 12 months all where negative.

By 12th month i have hiv blood strip test from one of the hospital it came negative.

Brief Answer:
Phobia - muscle spasms

Detailed Answer:
Hi dear friend.
Thanks for your query and an elucidate history chronologically.

Well, one thing is for sure , you are worried about something, which is not present and was not possible. We call this as phobia.
The first thing is stop doing HIV tests. Hand-jobs are not known to give HIV.

It is possible that the tensions / phobia you are carrying have caused the muscle of the body to go into spams, the armpit muscles seem to get most affected. This is a reflex process. Once you are out of the tension the muscle spasm would go as with it the irritation too.

I hope you do not have any swellings or skin changes under the armpits as you have not mentioned. If there is no local problem and the pain is muscular you may take the anti-inflammatory medicines.

I do not think you really need any tests at the moment. At the most you can visit your Doctor just to confirm there is no local problem

Brief Answer:
Apply liquid povidone iodine locally.

Detailed Answer:
Hi.
The armpits skin is different, almost always covered, sweat here is different. So there is a possibility of some sort of dermatitis which is not evident to Doctors.

I would suggest you to clean the affected twice daily with plain water and apply liquid povidone iodine 2 to 4 times daily. Keep the area well ventilated. Use loose shirts/ T-shirts to keep the area ventilated. I hope your problem will be solved. The pain will settle.

IF you think so, simply get the other STD tests done to keep your mind off this problem.

I hope this helps you. You will be fine very soon.

Brief Answer:
Anxiolytic medicine...

Detailed Answer:
Most welcome.

You do not have to worry about HIV at all at the moment.
Once the local condition of the skin in armpit is improved, pain will automatically go away.
You have noted this to be a ''mild pain''. So no need to take medicines for mild pain.

You may please request your Doctor to prescribe mild anxiolytic medicine which will reduce the pain and irritation. ( I am sorry that since this is a prescription medicine I can not write here).

You will be fine soon.

thanks doctor ..also during this period i have started seeing some of stretch marks around the armpit area and these stretch were also irritating a bit ,I could relate a bit based on your answers ..but just to confirm do you think it may be due to muscle spams.

so doctor, regardless of any thing do you think i don't need to worry abt hiv for sure and my tests are conclusive , no need to repeat the test anymore.

And my physician requested me try advil(pain killer ) for few days so shall I go ahead and try it.

Please advice.

Brief Answer:
absolutely nothing to worry about...

Detailed Answer:
The stretch-are necessarily physiological and can occur due to loss of fat and connective tissue. This is not a disease. Yes, Due to stretch-marks there can be retained sweat which in turn can cause irritation.

Muscle spasms again are most of the times due to anxiety only. And I have this feeling which I noted in the previous answer- which you have very well noted.

As I said hand-job alone can not cause HIV. So forget this at the moment. Do not repeat any tests at the moment. First come out of the problem.

Do you really think you need Advil? I am sorry to ask you a question , but still to confirm. Try avoiding such medication if the pain is mild.

Brief Answer:
Please tell about the pain in details.

Detailed Answer:
Hi.
Thanks for your feedback.
I would like to know where exactly is the pain? What is the severity ( mild you say/ moderate or severe sometimes)? What is the character-burning / throbbing/ just an ache ? Is it related to some movements of the neck or arms or a specific movement?

I still hope the pain will go off once you are tension-free.

You may go for one more test after 6 months just to clear your mind and tensions if you still have. I am trying to tell you that if everything is getting better why do you want to fear so much that it is affecting the ''present'' and by thinking about the ''past'' and ''future'' you may be having more troubles which otherwise would not have been there. I hope you are getting my point. Handjob was the only exposure and this not not known to give STD.

Thanks for the reply doctor.

pain is exactly on the armpit area its paining(moderate and it will pain in one particular area and it will go away again it will pain in some other place and sometimes i will have irritation in on joining place of arm and hand in area where chest muscle and hand join) in different places (but around the armpit only) after speaking to you i could relate it i could feel nerves were enlarged sometimes and irritating that might be because of muscle spams.

Last two days its fine no pain but an fear sitting in my always like i feel its going to pain again. Also i started seeing stretch marks during this time it starts from inner arm pit and running down little bit on the hand( i feel like some times its irritating)

Last week i have visited a dermatologist and explained him everything , he looked into the marks and examined my armpit and said its nothing and everything is normal.

By last 2014 December i have taken a master checkup including hiv test, they said i am alright.

Now i don't see pain for last 2 days but i have a fear its going to back its there in my mind. But your the first doctor said exactly about the muscle spam.. thank you very much doctor.

Brief Answer:
No more anxiety please...

Detailed Answer:
I am relieved to read that you do not have pain for the last 2 days.
Yes, you have noticed correctly that there is nerves - in fact the muscles fibers were enlarged.

Please do not worry about what will happen in future. This anxiety is one of the most important reason of your present problem

It is good that your had a good interaction with the dermatologist, to get cleared many doubts. Also nice to know about normal master check-up.

No tensions , no anxiety is the root of a normal life. boss.. So no more worried.
All the best
