Question: Dear Sir/Madam,
We have done Interval Growth Scan on 27 weeks aattach the reports for the same.Request you review and provide the feedback.
We are concerned about observation Liquor is Increased AFI-30(Polyhydramnios)
Wanted to check what is the normal range and what can be done in order to bring back to Noraml Level.
Does Polyhydramnios have any risk or complication during Pregnancy.

Hi,

Increased amniotic fluid or polyhydramnios is defined as Amniotic fluid index more than 25.

The normal range is between 8-25.

Increased AFI can be due to gestational diabetes, any anomaly in the fetus or idiopathic (no cause found).

A glucose tolerance test is advised to rule out gestational diabetes.

Anomalies are likely to have been ruled out in anomaly scan at 18-20 weeks.

Polyhydramnios can lead to preterm labour.

So diagnosis should be confirmed and further treatment should be done accordingly.

Thanks for information Madam.We have already done GTT test results are in Normal range as attached in the report.
Anamoly Scan did not reveal any anomalies and all observations were normal.
As you mentioned it can lead to preterm labour. How many weeks preterm labor can happen.
Will it be possible to determine the root cause for this and provide the treatment.
Where it would have any other pregnancy complications?
Can excess fluid can be removed?

Hi,

Preterm labour can occur at any time before 37 weeks of gestation.

If the anomalies and diabetes are ruled out, then other cause can be an infection like TORCH.

The rest for detecting TORCH infection should be done.

If negative, then the cause is unlikely to be determined.

Treatment can be given, if clinically indicated, by indomethacin or amnioreduction.

Consult your OBGYN specialist for proper examination and appropriate treatment timely.

Thanks for information.Our doctor has suggested to do NST scan wanted to take second opinion before doing the NST test.
Also is it ok to go for work after having Polyhydramnios does it increase if you go to work or rest should be taken in order to control Polyhydramnios?

One can work if there is no respiratory distress, or discomfort due to increased abdominal distension.

Work has no effect on the amount of amniotic fluid, however chances of preterm labour increase if there are jerks and exertion during work.

The decision should be taken according to the type of work to be done.

Thanks for your suggestions on this.
Wanted to take second opinion on this before we proceed.
Doctor is suggesting that to give steroids so that babies would develop good even-though preterm labor happens.
Wanted to confirm based upon your experience is it preferred to give steroids when we would be expecting preterm labour due to increase amniotic fluid during Twin Pregnancy ? does it have any side effects on babies or Mother?

Hi,

Steroid should always be given whenever there are chances of preterm Labour due to any cause. It has no major side effect on the baby or mother.

Duphaston can be safely taken during the third trimester of pregnancy.
