Question: Hello, in continuation to the above question. My fiance has got her periods on 25th March now and having her normal periods for last 4 days. We hope the periods must have got delayed due to intake of ipill and we understand now no need of going for a pregnancy test. Please suggest.

Hello Doctor, explaining the situation once again. I and my fiance had unprotected sex on 19th XXXXXXX and then she took ipill on 20th XXXXXXX within 24 hours. Her period cycle is 10th of every month. In January she got her periods from 10th till 15th Jan'16. So after taking ipill on 20th XXXXXXX there was no bleeding. She missed her next period which was due on 10th feb. We went for a HCG pregnancy blood test on 11th Feb (After 23 days of unprotected sex) and got the reports next day and the result was negative and then she also had periods on 15th Feb (2days after we got the report) i.e. periods were delayed for 5 days. She had periods till 24th Feb which lasted little longer than normal. As understood the cycle has changed to 15th now. But in the next month i.e. 15th March she did not got her periods. She got her periods now on 25th March and having her normal periods for last 4 days. We hope intake of ipill would be a reason for delayed periods and we also hope there is no need to go for pregnancy test again now. Please suggest.

Thanks for the query to HCM for an opinion.

I have completely understood your concern here and the fact that hcg based test has been negative last month and as menses have resumed by 25th march this month , pregnancy is completely ruled out and there is no need to get a repeat pregnancy test done at all.
The delay in menses has been due to side effect of i-pill intake which is known to delay next 2-3 cycles and that's common. Please avoid taking i-pill frequently , instead for contraception, she can opt for low dose oc pills regularly if unprotected intercourse is desired in future.

Hello Doctor,
Thanks for your quick response. As mentioned that since the period cycle is disturbed now, when can we expect next periods. In Jan'16 it was 10th to 15th (which was normal and regular). After unprotected sex on 19th Jan'16 and having ipill on 20th Jan'16 (within 24 hours), in Feb'16 periods happened from 15th to 24th (duration was longer than normal and bleeding was also heavy for few days). In Mar'16 it was 25th to 29th (25th was heavy bleeding, then next the flow was normal and now spotting happening today). When can we now expect next cycle. Can it also cross complete April and can happen in early May? Can also new cycle will be 25th every month?

Also, wanted to understand about implantation bleeding. Which month or which day it happens after unprotected sex?

thanks for the follow up.
The next menses as per her LMP on 25th march should be around 25-27th april ideally , of course stress and other factors may play a role if happen to delay and if she doesn't opt for another ipill again. If the menses are heavier and further delayed beyond 30th april then she can opt for low dose oc pills for 3 cycles regularly to regularise the menses.

The implantation bleed can occur after 4-5 days post unprotected sex. After ejaculation , the sperm has to travel up to the ampullarf region of the fallopian tube where fertilisation takes place and then the embryo travels down to the uterus and attaches to the uterine wall leading to implantation bleed . This takes 3-4 days post fertilisation.
