Question: Hi Doctor, my friend (girl) had unprotected sex on Dec 12th 2 am, she took unwanted 72 pill at 65th hour (14th Dec). Her last mensus was on 29th Nov and her cycle is 30 - 35 days. She had few drops of bleeding on 19th Dec. Is this withdrawal bleeding or signs of implantation? Does this bleeding mean she is not pregnant?

Hi there, Welcome to HCM.

Bleeding after I-pill is a common occurrence and does not indicate pregnancy or no pregnancy.

Your friend needs to do urine pregnancy test 3 weeks after taking the pill to rule out pregnancy. No predictions can be made based on the spotting seen.

So do not worry, as I-pill taken within 72 hour is 99% effective in preventive pregnancy.

Thank you Doctor! Can you tell what are the tests for STDs that are to be taken for the male partner to ensure that he has not contacted any diseases post unprotected sex. Kindly mention the tests and the timeline to be taken. Kindly consider tests for all types of STDs

Do test for gonorrhoea,trichomonas, chlamydia and syphilis now.
Repeat at 3 months time.
Also do HIV(ELISA), HBSAG now and again at 3 months to know if conversion occurs.

Hi Doc, regarding my first question, my friend had spotting from 19th Dec 8:15 am to 20th Dec 2 pm and normal flow till 20th Dec night (bright red with blackish dots), from 21st morning till now she is having heavy flow. She says she usually changes 4 pads during her normal mensus, but today she had to change 6 pads. Are you sure this is withdrawal / breakthrough bleeding? or is this her normal mensus (last mensus date was Nov 29th)? or is this miscarriage - is it possible to bleed so early if you miscarry (Date of unprotected sex 12th Dec)? She is refusing to check with a Gyn doc in person. Please advice

The first period after taking I-pill could occur early, be heavier than usual period. This is considered alright and need not worry you. It is good that she has her period as it means she is not pregnant.
It is too early to call it bleeding of miscarriage. Moreover I pill prevents pregnancy to occur, so the question of miscarriage does not arise.
She should not worry at all especially since she has got her period.
