Is Pulmonary Balloon Valvotomy A Safe Procedure?
Question: Hi Sir,
Myself XXXX and my wife name is XXXX and the discussion regarding my wife health issues.
Actually My Wife having the “CONGENITAL HEARTH DISEASE “ by Birth – Severe Pulmonary Stenosis and due to this she is not facing any issues till today. She is like normal women from birth for all the activities. And we have checked with lot of doctors for the opinion about this and everybody is clearly suggested to go with Balloon Surgery as normal since our condition is that she is becomes very normal like all others no symptoms till now and hope will not be any in future too.
So we are bit worrying about go for the surgery since my wife is afraid very much and she is not at all ready for coming surgery and she is telling very strictly that I will undergo operation after my first pregnancy that she want to be mother first then only she is ready to go for this further treatment for heart. So we have checked with some of the doctors with sharing the same information and some of the doctors are advised to go heart treatment first and then plan for pregnancy and some of the doctors are telling if she felt like this so you can go with pregnancy with consulting of Multi-Specialty hospital so we are planning with looking in to Multi-Specialty hospital with 1st Pregnancy ( That opinions from the Cardiologist only ). So for this case this is the second time we will discuss with Gynecologist for this suggestion so your suggestion will be more and more valuable for further progress.
And other issue is that July-2012 she got conceived and we started a regular checkup and exactly 3 months ( 11-Nov-2012 ) the baby got aborted automatically. Before 2 days ( 09-Nov-2012 ) we went for the check up with Fortis Hospital and that time we have told our Gynecologist for the heart issues also like from birth then doctor has prescribed to meet the heart specialist and need an opinion so the same day we met the heart specialist and he wants to take ECG and Echo test the same day ( 3 Months Pregnancy ) period even I asked the doctor that is that any harmful for the baby for this tests? But Cardiologist told no problem for the baby so we have taken all the reports on the same day and after 2 days automatic abortion happened so mentally my wife has the opinion that may be due to the ECG and ECHO test only the abortion happened so what she is feeling now that since some of the doctors are suggesting that pregnancy ( delivery ) with Multi-Specialty hospital will be better so she wants to go with Multi-specialty hospital after got conceived and after the delivery she is ready to go for heart treatment may b 1 year or after that.
And last 17 Months I was in ONSITE due to some official requirement so we were not in the situation to plan baby last 17 months after the abortion and I came back to XXXXXXX just 2 months back so now we are planning for baby so we decided to go for consulting the Gynecologist before planning since last time abortion happened so we decided to go with Cloud Nine Hospital XXXXXXX with Dr XXXXXXX Kini and 2 Weeks back we met him and he has suggested some reports all we have taken and according to our discussion and the result of the reports he has suggested to take the Tablet – FOLIC123 and OBIMET 500 since he has told that my wife is not having any other issues and but we have not told the doctor that she has one more problem for Heart Issues ( Severe Pulmonary Stenosis ) since the reason I have explained earlier. And We are very happy with Dr XXXXXXX Kini consultation but I feel bad that we are not informing the heart issues with him and since he is not aware about the heart issues he prescribed for OBIMET 50 and FOLIC123 so that’s why I want to know from you sir the real causes for OBIMET 500 and FOLIC123 that and she can take this tablet or not and will it be affected by her anyway since she is having this heart issues? Since you also one of the Specialists in Gynecology so I am sharing all the correct history to you and please suggest can we continue this tablet or not?
And herewith I am attaching all the recent reports and old reports (2012 ) and heart issues reports everything so please do the detailed analysis and please provide your suggestions since I want to know exact reason behind this history since my wife not ready for the heart surgery before pregnancy so without her knowledge firstly I want to know from you causes and suggestions and your great and valuable opinions for going forward.
1) OBIMET 500 and FOLIC123 – Will it create any problem to her heart or any other problem to her any kind of health issues ? If any please suggest we will stop the same and further suggestions.
2) Since Many Cardiologist suggests that we need to get clear opinion with Gynecologist also so please suggest can we go with Multi-specialty hospital for start pregnancy before the heart treatment? – Suggestions for everything please.
3) If we go pregnancy with Multi-Specialty hospital, what are may be the problems will come till the time of delivery? – Suggestion Please
4) What may be the clear reason for the older abortion?
5) And what kind of further suggestions needed from you for the better pregnancy?

Firstly i must tell your wife is lucky to have such a caring husband who is well aware of everything related to her medical issues.
After reading the history and studying all the attached reports i would suggest you to first get the pulmonary balloon valvotomy done d and then plan for pregnancy because of the following reasons -
- it is the simplest procedure in cardiology with minimal complication and is just like a minor OPD procedure. She will be discharged the same day with no further restriction of daily activities.
- in case pregnancy occurs with this lesion uncorrected then there are high chances of hospital admissions for whole of the pregnancy as stenotic lesions worsen during pregnancy.
ECHO and ECG are harmless procedure with no effect on the health of mother and fetus so do not keep any reservation about them.
These tests are done many times during pregnancy in cases with new onset heart disease or in case there is a risk of heart problem in the fetus.
ECHO uses the same principle as is used in ultrasonography i.e. ultrasonic waves so totally safe in pregnancy as is ultrasound.
The reasons behind abortion in her previous pregnancy can be many but most common can be presence of some chromosomal or congenital abnormality which is incompatible with life so nature tries to eliminate the fetus in its own course.
Still if she insists for pregnancy before surgery for heart disease then
- There are high chances of cardiac failure in later months of pregnancy
- She has to consult a good multispeciality centre with cardiologist and ICU facility
- there are high chances of cesarean section
- early termination of pregnancy may occur due to compromised status of the heart.
She has an element of PCOD as suggested by the reports of LH and FSH and for that she has been given Obimet. Folgel is a part of multivitamin family and routinely given in patients desirous of fertility. Both these drugs donot affect cardiac status so don't worry about them.
But in general never hide any information from the treating doctor in future.
