Is Rabies A Curable Disease? Is Tetanus Injection Required Along With Rabipur Vaccine?
I have few question about rabies:
1. Is rabies curable if person unfortunately developes disease.
2. In october 2012, rat bites me though it was a very minor one,my skin was alright and not broken,as i was unaware that rat also spreads rabies,i took rabipur vaccine 1 month later when 1 came to know.I took vaccine on 0,7,14,28 and 90 days that was ended on 25th february 2012. Last week one stray dog licked my leg so again i have taken 2 doses till now on 0 and 7 day. My question is that do i need to take further vaccine or not
3. Is tetanus injection required along with rabipur vaccine.If yes then how many doses ands when.
4. Is there any side effect od taking more rabipur vaccine.
Please respond to this as i am very worried.

I am outlining the answers for your questions in stepwise manner , go through them .
1Q. Rabies is a viral disease , and it's non-curable disease , if the patient acquires disease , the treatment is purely symptomatic . There is no specific anti-rabies medications. The patient develops encephalitis and dies due to respiratory failure.
2Q. Just licking of a stray dog on a intact skin with out abrasions , skin cuts or any skin tears , doesn't cause transmits the disease. Because intact skin doesn't allow the virus into the body. As you have already been vaccinated , No vaccination or treatment is required for this , just washing the licked area with detergent is sufficient. But keep in mind licking on the eyes, lips or oral cavity should require vaccination because dogs saliva directly comes in contact with our secretions.
3Q. Tetanus injection is required in street dog bites , when there is breaking of skin in any form like if its a XXXXXXX bite or a abrasion or a laceration . Tetanus injection is given to prevent tetanus disease not to prevent rabies or wound infection. Tetanus can be given simultaneously along with rabies vaccine but on a different site.
Tetanus is given as single dose , if the patient is immunised already during child hood with DTP vaccine ,single dose is sufficient , and this single dose gives protection upto 10 years.
4Q. Rabipur vaccine is a chick embryo vaccine , it has some antigenic effects , if you are allergic to egg protein then you will be allergic to this vaccine. The symptoms include itching, urticaria, chest tightness, angioedema. If you are allergic , then these symptoms would have appeared already at the time of your first dose , so nothing to worry now.
There are no side effects reported on overdosage or excess of injections.

I have few questions more:
1. I have received tetanus single dose 2 months back. Do i require the same in case of future dog bite. If yes then tell me with which rabipur dose schedule it will be needed and how many tetanus injections.
2. As i am already vaccinated, though for a safer side, in case of future dog bite,how many rabipur injections to be taken and on what schedule.
3. Upto how many days we have to watch the dog's behaviour (whether street or pet ) in case of bite. If dog is alright should we stop vaccine. And if dies, besides rabipur what addition vaccine do we require.

1Q. In case of future dog bites, if you are bitten by a dog within 3 years, no need to take any tetanus vaccine again. If it is more than 3 years over since you received the vaccine, you should take one booster dose of tetanus vaccine.
2Q. After full course of rabipur vaccination, it gives protection for 5 years, if a dog bites with in the period of 5 years, only booster dose of 2 doses on 0 and 3 days is sufficient. If a dog bites after 5 years of full immunization, you need to take full course of 5 doses again.
3Q. According WHO, the dog should be observed for 10 days about any mad, erratic behavior or any death. If the dog is alright after 10 days, and you are sure about it, then you can discontinue the vaccination. Most of the doctors recommend full course of vaccination because we cannot watch the stray dog continuously for 10 days.

1 if dog dies after 10 days and rabipur vaccine is going on. Beside rabipur vaccine, any additional vaccine is required or not or we have to go with the flow of rabipur vaccine only.Beacause this case is very XXXXXXX as surely the animal is rabied one.
2 :Can you tell me is rabies virus exist in dried saliva also??
3 : Also can rabies happen to human to human through infected saliva??
4 : Beside saliva, can rabies exist in blood, urine, sweat or any other liquired or tissues or body part of the infacted animal or human.

The answers are as follows.
1Q. Nothing specific is available to take , except being vaccinated, if the dog dies with in 10 days. One thing we can administer is HRIG -20 IU/kg (human rabies immunoglobulin), along with the 1st dose of vaccine (half dose to gluteal region and half dose to bite area), if the bitten area is very XXXXXXX and severe lacerations occur.
Even this HRIG ineffective if you give it after 10 days. Apart from this, nothing is there to give.
2Q. Rabies virus doesn't exist in the dried saliva, as it will be inactivated and died after exposure to outside environment and temperature.
3Q. Rabies from human to human , definitely occurs through saliva, there is no doubt on that. Same like with animals.
4Q.Rabies virus exists only in the salivary glands of either animals or humans, when they are infected.

1. Can be there is any variation in the schedule 0,3,7,14,28 if person misses any dose. Suppose if i missed the 3rd day dose,can i take it on 4th or 5th day or after 28th day.or i have to shift the days of other doses accordingly.
2. on the day when dog bite. if someone delayed to take the vaccine, says if he takes vaccine after 1 week later then the same schedule is there or to compensate that lapsed time some additional dose to be provided or not.
3. in case of pre- exposer due to risk, what is the schedule of rabipur vaccine and is the same effective for 5years and we have to take doses on 0 and 3 days if dog again bites after pre- exposer or again we have to switch to post exposer as this is the case from pre to post.

My answers are as follows,
1Q. If someone misses the prescribed dose of rabipur vaccine, we advise them to take it as soon as possible, if someone missed the dose on 3rd day, then we advise him to take the very next day or day 2 without affecting the next succeeding dose. Most of the times patients will be restricted to the schedule because of fear.
2Q. If the person delays to take the vaccine as soon as possible after dog bite. He is on his own risk, because once the infection sets in, even if you give vaccine, there is no use. Vaccine helps to produce immunity against the disease; it does not cure the disease progression. Also there is no separate schedule or change of dose of the vaccine.
3Q.The general recommended pre-exposure prophylaxis is 0, 7, and 21 days.
If a person had immunized before (i.e. pre-exposure prophylaxis) and if he is bitten by a dog, he has to follow booster doses of 0 and 3 days only. If the person is not immunized before the bite of a dog, he has to take full course.
Either it may be pre or post exposure prophylaxis gives protection upto 5 years.
If the person is very high risk person like veterinary doctor or a paramedic working in a lab with rabies or persons who handles the wild animals, the protection period should taken as 2 years only.
