Is Surgery Advisable For Left Shoulder Impingement Syndrome?
Chief complaints/reason for admission:
This is a 43-year-old male member, who presented to the hospital with complaint of left shoulder pain.
History of present illness:
• The patient has been experiencing the left shoulder for 3 months.
• The pain increases with overhead movement.
• The pain gets worse at night when he is sleeping over the left shoulder.
• There is no history of trauma or injury.
• He was placed on non-steroidal anti-inflammatory drug and topical gels, but without any improvement.
Physical examination of the left shoulder revealed,
• Local left acromion tenderness.
• There is positive crank test.
• There is limited range of motion in abduction and internal rotation in flexion.
X-ray of left shoulder AP, outlet and 300 caudal views:
• Bigliani type II acromion noted.
• Bone mineralization appears normal.
• The remaining visualized bones reveal normal morphological features.
• No evidence of fracture or dislocation seen.
• Acromioclavicular and glenohumeral joints appear normal.
• No evidence of soft tissue calcification seen.
Past medical/surgical history:
His past medical history is otherwise remarkable for anal fissure, for which he underwent fissurectomy.
Diagnosis:
Left shoulder impingement syndrome.

I have gone through your query and understand your concern. You have got impingement syndrome. It is due to Bigliani Type II acromion in which acromion is curved. It is treated by rest and analgesic such as ibuprofen for pain relief. Physiotherapy and local steroids are given for relief. Sometimes Surgery is required - the goal of any surgery to reduce the effects of impingement, by increasing the amount of space between the acromion and the rotator cuff tendons, which will then allow for easier movement and less pain and inflammation.The operation performed is Arthroscopic Subacromial Decompression (ASD) . In this portion curved portion of acromion which is responsible for impingement is removed.
