As an Urologist,i can fully understand your concern.
The sperm count is sub-optimal,with lower than normal counts and motility.
There's a minimal infection seen,which can be treated by antibiotic course.
The minimal sperm count should be, a minimum of 20 million/ml of semen.
Also,the minimum motility(movement),of sperms should be more than 50-60 %.
Your report shows active motilitly of 30 %.
Most Urologists would insist on a second semen analysis,with a 2 weeks gap.
Two to three days abstinence before the test, is mandatory for a reliable report.
You'll need a repeat semen analysis test and then decide on further treatment.
