Question: Hi, Dr. Spaho. I did take your advice and addressed some dental issues prior to botox, etc including one extraction and due to bone loss, oral surgeon said a bone graft was definitely needed. Not sure if I should have allowed this since I believe the bone was merged into the open space in the jaw and it made the TN2 intensity hit the roof. Of course, this cannot be undone and from a clinical view, was needed. Knowing how the nerves work, just want to get your feedback if you feel this procedure did irreparable damage in terms of this pain with TN2 or will bone placed there eventually calm down? Thank u so much.

Since there is some bone missing, we may think that the nerve ends of the gum are more exposed to minor trauma, pressure, etc.

So, if the bone graft is placed to fill this bone gap, it may eliminate these minor trauma and eventually lower the intensity of the symptoms.

So, in my opinion, it may help.

Hi, doctor. I am glad to hear this may help but may be long term. In the short term (at least next few months), due to the trauma of the surgery in the jaw, would like your opinion on meds since the throbbing is very bad and teeth/gums are all lit up with pain. I have several here (Neurontin, Lyrica, Cymbalta, Trileptal, Amblify, and Elavil) Would like to focus on just one maybe two. For these specific symptoms and considering the trauma recently done in jaw, which medication(s) would u feel would offer most relief? As always, thank u.

In my opinion, Lyrica combined with a short-term anti-inflammatory drug (ketoprofen for example) is a correct combination fur such symptoms.

Lyrica may be used for a few months, the ant-inflammatory drug may be used for a couple of weeks.

Discuss with your Doctor these issues.
