Is Vaccination For Japanese Encephalitis Safe And Effective?

Question: My daughter and i are travelling to southeast asia next week (leaving 6/17) but in our excitement to make travel plans we completely overlooked the need for vaccinations. While we have gotten hep a and b and typhoid injections, I am very concerned about japanese encephalitis as we will be travelling to siem reep cambodia and bali indonesia.

I can get a single shot of the (very expensive) vaccine but does it make sense to? Will we gain ANY immunity before reaching cambodia on XXXXXXX 22 (5 days after our original innoculation) I dont mind paying the money for the peace of mind - if it will make a difference but i am also concerned about being on a plane for so long and coming down with side effects while enroute. This is making me very anxious and if i didnt have so much money to lose i would probably cancel the trip altogether.

Ive already read every piece of documentaion i could find online so please dont quote cdc recommendations or the general statistics for side effects et al. from what I have read clinical trials were run on less than 5,000 patients and the new vaccine hasnt been in use for very long so the track record is short.

Please respond if you have real first hand knowledge of infectious diseases and characteristics and efficacy of this particular vaccine.

Cell culture derived live attenuated vaccine for Japanese Encephalitis is quite effective in prevention of disease. The efficacy of vaccine starts in couple of days after injection.
I am from Republic of XXXXXXX and resident of XXXXXXX State. Japanese Encephalitis is quite prevalent in some of the districts. During outbreak of cases of Japanese Encephalitis , vaccination is quite effective in preventing the disease in susceptible individuals. There are other types of vaccine available, but Cell cultured live attenuated vaccine is most effective. The vaccine is 98-99% effective in prevention
If the duration of stay is more than 4 weeks in Endemic/Epidemic area vaccination is indicated. Also would advise you to go ahead with vaccination. The only common adverse effect of vaccination is redness/erythema at site of injection of vaccine. I would recommend you to take anti-inflammatory drug like Ibuprofen. There is no reason to be worried about in flight.

Are we talking about the same drug? The only form of vaccination in the United States is Ixiaro an inactivated Vero cell culture–derived vaccine manufactured by Intercell. Is this the one you are referring to?
Is it very effective even on a very short time schedule because this not at all what i am hearing from US practitioners. They say there is very little immunity within the first 28 days and until the second shot, at day 28 it is not worth getting.

Further they say that side effects are problematic. Are US doctors ill informed? Overly cautious? or does our centers for disease control offer inaccurate information?

Dear Ms,

We have equivalent product manufactured by our XXXXXXX firm that is Vero cell derived Inactivated Japanese Encephalitis vaccine.
Wide Phase III clinical trials have already been conducted. Whenever there is an epidemic , administering of this vaccine has been very much effective in preventing disease.

There is difference between sero-conversion and protecting against development of disease.
In the clinical trials, the vaccine produced by XXXXXXX Council of Medical Research(ICMR) showed superior safety and immunogenicity, in comparison to live vaccine. It met all its primary and secondary endpoints in the age group of 1-50 years, after 1 or 2 doses in vaccination. Phase III trials have shown 80% seroprotection rate within 1 week after first dose, 98.7 per cent sero-protection after 28 days, and 99.8 per cent sero-protection 28 days after the second dose.
Even after the first dose , there is significant level of sero-protection. The second dose of vaccine acts as booster further increasing the efficacy.
As I have mentioned you, the common adverse effects which we clinically come across are pain at injection site and mild myalgia. From the protection aspect its quite effective which has been demonstrated in clinical trials here in XXXXXXX

We routinely come across several clinical cases of JE, even in the same household if one individual is affected by JE immunization of other individuals of same family have prevented clinical disease in them.

As you are travelling to Endemic area, the benefits of vaccination out weigh the minor risk of few adverse effects. Coming to the cost factor, I am not aware of price over in US. But in XXXXXXX market its available for INR 160 that is less than 3 USD.
If you are going on trip to South East Asia, I would recommend you to take the vaccination.

Dear Dr. XXXXXXX XXXXXXX Kaltari, Thank you for your extensive response. I appreciate your time and expertise in helping me make sense of this concern. By the way, here in New York, we are being charged $300 per person per vaccination for a total of $600 per person for the full course of treatment. Quite a difference in price I would say!!! We may forego the second shot under the circumstances as we will have resturned from our holiday before the second dose is administered in 28 days. Is there any evidence that an accelerated rate of vaccination has any value say at 0 and 7 or 14?

Let me explain you again.

There are many Endemic areas In the State where I reside. When there is outbreak of cases of Japenese Encephalitis , if ten families are affected in a given geographical location. If one person is affected in each family with total number of people on a average being six. The other five individual of given family are at very high risk of getting the infection. Immunization of such individual has prevented from any clinical disease occurring in them.

The cost of vaccine is definitely very much over there. As I mentioned you as per clinical trials the seroprotection rate is near to 80% in phase III clinical trials during the first week which increases to 98.7% 28days after 1st dose. So taking vaccination is definitely beneficial instead of foregoing it taking into consideration of the cost. I have not come across any major adverse effects after the administering vaccine.

We leave shortly for a New York City Clinic that will administer our shots. I think they will try to talk us out of the vaccination but based upon your information, we will be having the shots anyway.
