Question: Hi Doctor,
My daugther is 6 years 9 months old.
She had fever 3 days back with some headache and stomach pain(mild) and fever came down in 2 days. From 2nd day rash started developing on feet and hands palm. She is active no fever, no pains but rash is not gone even after 2 days. Went to see a doctor and went for a CBP test. WBC(4500), MCV, MCH, MCHC are slightly low and other parameters are normal in range. Doctor suspecting HSPC or small vessel vasculitis if not normal fever rash as Dengue test is negative. We are more worried now. It could be due to serious problems? what is HSPC. Please suggest what to do

Brief Answer:
Seems like foot hands and mouth disease.

Detailed Answer:
Hello,
I read your query and here is my advice.

I see the pictures you attached and it seems more like foot hands and mouth disease.
The blood count is normal despite mcv which isn't much significant for this situation.
You didn't mentioned any mouth pain or rash in mouth but you can ask her if she feels pain in mouth or look it she has rash in it.
Also the two days of fever and then the rash in hands and feet seems more like hands foot and mouth disease rather than HSP. In case of HSP the rash appears after 2-3 weeks of fever, headache and muscle pain.
The rash is in HSP appears in legs , joints, buttocks knees and elbows.
If it is HSP it may cause arthritis or kidney problems including blood or protein in urine which in most of cases go without major consequences.
You may need to do her urine analysis to check for protein or blood in urine.
Hope I have answered the question.
Let me know if I can assist you further.
Thanks.

Hi Doctor thanks for the response.
She doesn't have any rash or pain in mouth. Also no joint pains or any other pains.
First day she had headache and mild stomach pain with fever and gone along with fever.
Rash started when we started giving her some pan related syrup doctor prescribed for stomach pain. we thought its an allergic reaction and stopped giving it. Later observed rash start reducing in one place and few dots in new places on foot and hands. thats why we went to doctor for blood tests.
Urine examination is a must for her now? We are really worried and tensed from the moment doctor said suspecting HSP as it is very dangerous disease by the images and posts online we saw. Please suggest.

Brief Answer:
Monitoring her symptoms

Detailed Answer:
Hello dear, thank you for asking again.
HSP is more dangerous since it can cause kidney damage that's why I recommended doing urine analysis often to monitor her.
According to the pictures and the symptoms you are referring it doesn't seem like HSP but you should not neglect since your doctor suspected it.
Giving her acetaminophen for pain and inflammation is advisable. Also keep her hydrated giving more fluids. Monitoring is the only thing you can actually do.
Let me know if I can assist you further.

Hi Doctor,
My doctor said if the rash is not going down in two days or if it is increasing then we will go for further tests. Now we observed a little change like its reduced now and no new rash after started using Atarax syrup. Do we still need to go for urine test? How long the rash last if it is HSP? Will HSP go on its own without any medication also? Now no fever, rash coming down, no aches. Still we can suspect HSP?

Brief Answer:
See details below.

Detailed Answer:
Hello dear.
If it was HSP the rash was not supposed to clear so fast but it may last for weeks or a month.
If the rash is going you don't need to do further examinations to her .
The HSP can also go down without any specific treatment .
Let me know if I can assist you further.
