Question: I've been dealing with a rinithis problem for the last two years but o still don't know if it's allergies or not. My face as really changed because of this, I have deep dark circles and malar bags, I look old and tired but I'm only 25 and it makes me really depressed. Will my face go back to normal if I manage to control my current condition?

Brief Answer:
Proper treatment of sinusitis is necessary and some other conditions need to be ruled out.

Detailed Answer:
Hello and welcome to "Ask a Doctor" service,

I carefully read your query and understand your concern.

Chronic allergic sinusitis may cause malar bags due to eyelid swelling that may associate it. Also, patients with chronic sinusitis may have obstructive sleep apnea which causes them difficult sleep and tiredness which further contributes to malar bags.

I would recommend you to have proper and regular treatment for sinusitis. Such treatments would be Loratadine, Fluticasone nasal spray, nasal lavage with a normal saline solution to clear the nose. A room humidifier may be used if the air in your room is dry and you will need to avoid chemicals or environments that worsen your sinus symptoms.
If no improvement is noticed with a similar treatment you will need to make sure there is no nasal polyps or nasal septum deviation. In these cases, surgery may be required to open the nasal passages and improve symptoms of chronic sinusitis.

To rule out speel apnea you will need some sleep study (polysomnography). Alcohol and smoking can make malar bags worse as well.

On the other hand, other causes should be ruled out such as thyroid disease, liver, kidney, and heart conditions, and the following tests may be recommended: TSH, CBC, creatinine and electrolytes, ALT, AST, Bilirubin, blood levels of albumin, etc. Your doctor will check your heart and lungs during the examination as well.

I hope this answers your query. I remain at your disposal in case further medical assistance is needed.

Regards,
Dr. Antoneta Zotaj,
