Hello
Thanks for query and highlighting your problems in detail which you have since last 10 years .
The symptoms like dribbling after urination ,pain in perineal region,
painful urination ,and pain while ejaculating is mostly due to chronic infection of Prostate (Chronic Prostatitis ).
You need to consult qualified Urologist for clinical and Digital Rectal Examination (DRE) and get following basic tests done to establish the diagnosis .
1) Urine routine and urine culture
2) Semen examination and Semen culture
3) Ultrasound Scanning of abdomen and pelvis .
4) Culture of a Urethral discharge obtained after prostate massage .

Is chronic pelvic pain syndrome and pelvic floor dysfunction the same thing or could both cause the symptoms that I am experiencing?
I've been tested many times and there's no bacteria in my pee also I've been through 2 months of Tetracycline for acne which should also work on urinary infections as far as I'm informed.
How likely is it that my symptoms are caused by bacteria when they've been with me for 10 years + up to now and mainly come if I have an ejaculation or sit down a lot over a period?
I recently started having problems with my bowels and went to get checked for all kind of things but they never found anything and labeled it as IBS, could a pelvic floor dysfunction be a cause for all of my issues?
My biggest issues with the IBS symptoms are that I feel often like I can't fully empty out my bowel movement, like I have to strain or that my muscles don't have the same 'push' power that it used to have and then I continue feeling a need to go to the toilet but can't go and that causes a lot of irritation for me.
I've been checked for problems with my urinary issues many times but they never found anything and my pain usually is between scrotum and anus where it lingers for hours.
Do you know why peeing after ejaculation can minimize the pain?
Chronic Pelvic Pain syndrome and Pelvic floor dysfunction both are same
Detailed Answer:
Hello
Thanks for follow up .
Chronic Pelvic Pain syndrome and Pelvic floor dysfunction are one and the same entities and also called Chronic Non Bacterial Prostatitis .
Chronic Non Bacterial Prostatitis is a condition where the patient have all the symptoms that you have but there are no positive findings detected on relevant laboratory investigations.
It is the most notorious disease where patient and treating Dr both are handicapped (All the tests are being reported to be normal).
As regards IBS it is said to be due to Neuro Muscular imbalance in large bowel resulting into irregular bowel function of absorption of water and motility of large bowel giving rise to symptoms like tenesmus, loose motion or constipation and pain and discomfort in lower abdomen .The main cause for the same is a stress factor .It is also never cured completely and have relapses off and on .
The drug like Mebverine twice daily does help to regularize bowel function and gives relief to certain extent .
One has to modify diet habits like avoiding fibers in diet ,salads, milk products ,Oates, raw and junk foods.
