Brief Answer:
Specialists' opinions and investigations is the key

Detailed Answer:
First of all, the most important thing is to get a proper diagnosis.
This is possible by taking an opinion of Specialists like Orthopedic Surgeon for back ache, MRI of back and spine so that an exact cause is found and treated well.
Secondly take an opinion of a Gastroenterologist as the problems looks like colitis, may need colonoscopy and CT scan of abdomen to know the exact cause of pain in lower abdomen and thus get treated accordingly.
Another important point is best control of diabetes without which many a diseases will not come under control. Get an opinion of a Diabetologist for proper evaluation and proper management.

I hope this answer will help your son to get a proper diagnosis which is very important to get a proper plan of management.
Feel free to ask for further relevant queries if you feel that there is agap of communication.
Also give feedback once the investigations and opinion of Specialists are done.
