Dear Doctor good morning.Earlier i have contacted around 1 to 2 years back.
I am 78 year old male.I have some medical conditions which i have detailed and uploaded to this query.I am taking the medicines and and all under normal levels. No problem or whatsoever.
I take Betacard 50 mg , Deplatt 75 mg, amtas 5 mg Glycomet SR twice ,Urimax 04.mg and Thyronorm 50 mcg.Till 30th Dec 20 this is the regimen.On 31st December i have experienced orthostatic hypotension and the readings were given in the attachment. When i approached my doc he suggested me to reduce the dosage of Amtas from 5 mg to 2.5 and also to change the time of the dosage and shifted to night.So i have started taking reduced dosage of Amtas 2.5 after dinner from 2nd Feb 2021. After that day i have never experienced this hypotension.
I also under gone TMT in 2017 which has come negative and also an ecg in 2017.Again on 1st feb 2021 i have undergone ECG which was prescribed by my Doc.He said both the ECGS have no changes and they are normal.
What i wish to know from you is Whether my BP readings are normal? Whether amtas 2.5 suited to me?How is the latest ECG of 1st Feb ? Whether you suggest any other safest BP medicine in addition to the present Betacard ,Deplatt and Amtas?
Kindly go through the attachments and advise me.

Detailed Answer:
I hope you are doing well. I have reviewed the file attached by you and I would like to point out that your early morning blood pressure is staying slightly towards the higher side.

There are two things which needs to be done
1. Reduce salt intake in evening
2. Amtas dose needs to be uptitrated to 5 mg. Take it around 7:30 pm instead of 7 pm.

Rest your ecg reports seems alright with no significant variations.

doctor saab, my doc tor says amtas is the culprit in orthostatic hypotension and asked me to reduce the potency. If i take 5 mg again doesnt it recur again? if there will be no problem i shall start taking it. shall i monitor the readings ? if so for how many days?

Detailed Answer:
Sir, orthostatic hypotension is not drug induced hypotension. Orthostatic hypotension is when bp falls when patient gets up quickly on their feet from a lying down position. It happens due to venous incompetence and sudden gravitational pooling of blood in dependent part mainly legs.

dose increase is acceptable because your earring morning bp is staying more than 150 mmhg. Also keep monitoring your blood pressure as you are doing before for atleast a week.
