Dear Doctor,
The question is regd my wife.She is 70 yrs old.She has diabetes (type II 2008) and Mild hypertension (1996)
She has taken Amlodipine from 1996 to 2019.When she developed Swelling in ankles, my doctor asked her to stop amlodipine and start taking Cilacar (clinidipine).
Ever since she has taken cilacar from may 2019. i feel her bp readings are on the high side.However she has continued the drug
My phisician changed the drug from 27/2/2021.He prescribed Azelsartan medoxamil 40 mg.
I have monitored on Omron digital instrument and the readings as under.
27/2/21 6 pm 147/71
9/3/21 5 pm 154/70
10/3/21 12 pm 154/82
11/3/21 5 pm 170/73
13/3/21 10.30 am 169/85
Do 7 pm 150/94 at the

My phisician is a very well experienced doctor and a retired professor of medicine and also worked in Cardiology for nearly 7 years.
Yesterday he said that the her bp is normal and need not change the drug for some more time.
Please do not think that i have no confidence in him. I have it fully, but i am afraid to speak freely with him.
Though he said that her bp is normal i feel that she has high bp.Bcause when she was taking Amlodipine her readings always below 130/80.Eversince the drug is changed first to cilacar and then to azelsartan medoxamil 40 mg her readings have not come to 130/80 level. May be i am wrong.
I request you to kindly go through this and advise me wether her bp is normal and there is no need to change to a betablocker or a calcium channel blocker or any other good angiotensin drug. Does she has to change to a new drug or to take another incombination of Azelsartan ? shall i ask my phisician to change to a time tested drug like Telma40 or Telmisartan or metxl or metaprolol or Betacard Kindly give me a direction and a valuable advise.
One more doubt is why the bp readings do not come down to 120/80 even after taking medicines in elders unlike in middle aged men?

Apologies for delayed response. As per the latest guidelines, the recommended blood pressure for elderly diabetic patients is less than 140/90mmhg.
Keeping that in mind the bp readings of your mother are slightly higher (mainly her systolic bp) at multiple occasions which needs to be addressed.
Amlodipine works very well in elderly patients. Only prob is that it causes ankle edema after sometime. Cilnidipine is a good alternative for this. (Why cilnidipine was stopped for your mother?)
I would like to suggest that you should discuss with your doctor about the scope of starting Telma-h instead of azelsartan. Also dietary salt intake needs to be restricted.
