The MRI report attached suggests an old injury or wear and tear of the anterior talofibular ligament. This can be stress related tear and can be treated with rest an an elastic ankle brace. All painful activities need to be restricted.
The small ganglion cyst in the joint usually does not need any treatment if it is painless. In case it is causing pain or is increasing in size then local steroid injection can help but its recurrence is also seen.
