Question: Last week I hade an accident and part of a bathroom tile got pierced into my right hand pinky. A little part of the tile was left inside and the x rays shows that its close to the knuckle ( the proximal inter-phalangeal joint ). I've been scheduled for surgery on Wednesday to extract the piece. The doctor has indicated probable neurolysis and general anesthesia. I would prefer local anesthesia and would like a second opinion on whether general anesthesia would be necessary in this case. The first picture attached shows the piece of the tile that pierced the finger, the little missing part on top is the piece that got stuck. The second picture shows the wound. The piece is close to the knuckle above the wound.

There are no pictures attached with this query. To remove a small piece of tile in the finger, local anaesthesia is also sufficient. But in some cases it becomes difficult to search for a small piece in the tissue which may take time and general anaesthesia may be needed. In case the surgeon feels that the surgery might take more than 10 minutes then local anaesthesia will not help much. Let your surgeon decide the type of anaesthesia needed.

I have seen the attached images. There is a small chip of tile but time needed to search it in the bleeding tissue varies. It can be done under local anaesthesia also. Let your surgeon decide the anaesthesia best for you.

Just a little follow up. You said, "if the surgery takes more than 10 minutes, local anesthesia won't help much". Could you explain? I've heard that local anestesia can last for hours.

Not feasible for the little finger.

Local anaesthesia like spinal anaesthesia or a nerve block can last for hours. In the case of a little finger these are not feasible. Ring block can be given that too without adrenaline which usually lasts for 10-15 minutes. The duration of anaesthesia depends on the part of the body that needs to be anaesthetized.
