Ligament Reconstruction And Neurolysis Done After Accident. How To Overcome Foot Drop?
Question: I had met with an accident in 2008, My left knee was injured with torn LCL, and PCL. At the time of impact I had partial parnoial nerve palsy. In order to get cured I went through Faciotomy first, and then went for Ligament reconstruction for PCL and LCL. During the surgery I also went through Neurolysis. After the surgery, My Knee is ok, but I have still don't have proper nerve functioning from my ankle to the big toe and toe fingers.Thus have a foot drop, My foot has got jammed from ankle so the range of dorsiflexion and planterflexion is not there (to some extent it ie there). Currently I am taking no medication. No allergies as such. Please suggest how to overcome this, is ther any surgery for the same?

At present you have stiff ankle with foot drop. To overcome foot drop , you have an option of tendon transfer surgery. But before this your ankle joint should be supple. So firstly you need good physiotherapy of ankle to regain full range of motion.

Thanks for yor prompt reply, I had visited many physiotherapists, and they said that my ankle is completely locked now, and the range of motion cannot be brought back, as the anke was neglected for the past 2-3 years. They said that there's some sort of surgery where they make the anke supple in the first place and then do the tendon transfer during the operation. Is this possible? what re the pros and cons, how much do I have to spend for such a surgery? Thanks in advance.

It seems that fibrous ankylosis is the cause of your jammed ankle.Fibrous tissue and adhesions can be removed by the surgery.It can be performed by arthroscopy. It is minimal invasive and good procedure. For such surgery you have to spend approx 30-40 thousands.

Thanks Dr. XXXXXXX for the reply. So I can get the adhesion removed and tendon transfer in one single operation ? But, since my motor nerve is not working, would I be able to lift my ankle after the tendon transfer. How much is the cost for tendon transfer. What is the physiotherapy protocol to be followed after the operation? How long does it take to recover after the operation. Since I am working, I will have to think accordingly.

It is not good to do both the procedure in single operation.Firstly adhesion should be removed, after that good physiotherapy of ankle should be done. Tendon transfer should be considered after that.
You will be able to lift your ankle after the tendon transfer.It will again cost 30-40 thousands approx.
After tendon transfer it will take 2-3 months to recover.

Thanks for the suggestion, I can also see a little bit of bone deformity in my leg, since my ankle is not moving, the biones of my left leg (from XXXXXXX to ankle) have inclined a bit towards my right side, can this be a concern for me, would this affect the operation too?

This little bit deformity seems to be compensatory to foot drop and stiff ankle. It doesn't seems to be significant concern and this will not effect the operation significantly.
