Male Age-58 Height 6'3 Weight 315 I Had A CBC

I had a CBC test done this week. The RBC, HGB and HCT results were below normal. There was an IG# and it was 0.05 and reference range is 0.00-0.003. The IG % was 0.90 and the reference range is 0.00-1.00. This is on the upper end of normal. All other CBC results were normal and the WBC count was 5.8. The RBC count was 4.44, reference range is 4.5-5.7. The hemoglobin was 13.3 and the reference range is 13.9-17.1. Finally the Hematocrit was 40.6 and the reference range is 40.9-49.7. The doctor wants to repeat the test in a few weeks. He stated the low red counts might be due to some hemorrhoids that I have and sometimes I have excessive bleeding from these. He stated the high IG number is usually related to an infection. I asked him if he thought this could be leukemia and he said no. He stated that my WBC count would normally be higher than normal for leukemia to be present. I have researched this on the internet and as you know there are many different opinions. He did tell me if the count doesn't return to normal he would refer me to the GI doc. Note have a colonoscopy every 5 years and each time they find polyps but none have been cancerous. My last one was in April 2017.

Nothing much to worry.
Detailed Answer:
Hello,
Thanks for the query.
I have gone through your reports and I can understand your concerns.
There is only a slight decrease in hemoglobin and red cell parameters.
Most often it is due to deficiency of iron-rich foods otherwise knows as a nutritional deficiency.
There is nothing abnormal suggestive of cancers or leukemia.
As of now include more fiber-rich foods to your diet and repeat the test aster four or six weeks.
The colonoscopy does not show any suspicious lesion as well,
I don’t think you need repeat colonoscopy based on this report.
Thanks

It is not significant.
Detailed Answer:
Hi,
IG is immature granulocytes and they are almost in the normal ranges. It is a non-specific parameter and does not have much clinical significance. Variation may be seen with any infective causes.
RBC count may be decreased in any form anemia which may be nutritional causes or blood loss as in hemorrhoids.
Thanks
It is not significant.

1. Potassium level on current CMP test is on the high end of normal? Could blood pressure medication cause this or an underlying condition?
2. PSA test was 2.3 and the results 15 months ago were 1 for PSA. This means it more than doubled in the past 15 months. I did see a urologist this week and he wasn't overly concerned. He did a rectal exam and said the prostate was normal to slightly enlarged but again he didn't think the result being 2.3 versus 1.0 15 months ago was significant. He did suggest getting another PSA test in 6 months. I thought if your PSA test doubled or more than doubled in a year this would be a cause for concern but the Urologist doesn't think so in my case. I have doubts about his opinion.
3. My calcium sometimes is on the high end of normal. If you look at the current lab test from 8/31/20 it is at 10 which is pushing the high end of normal. My understanding is that the calcium level in adults should always be in the 9's range. Back on 5/24/19 I had a Calcium Ionized test done which measures calcium and PTH. This test was abnormal and I have uploaded the report. You can review the numbers but it suggested that I had Non-Parathyroid Hypercalcemia. The numbers were slightly out of range and again my doctor wasn't concerned and told me that with Hypercalcemia in your calcium levels would likely be 13 or higher. I just got some medical advice that contradicts that. There was another time since 5/24/19 where my calcium was 10 on a blood test that showed the high end of reference range was 9.9. The bottom line is my calcium always seems to be on the high end of normal and the Calcium Ionized test seems to indicate there could be a serious problem.
4. I am wondering if all of the test results with lower RBC counts, high potassium, high calcium and the abnormal Calcium Ionized test are related to a more serious underlying condition. My family doctor has always discounted these test results however is recommending repeating the CBC test.
5. I need an honest and intelligent opinion about all of the results and what it might mean.
Thanks, XXXXXXX

Detailed Answer:
Hi,
I will take one by one.
1- Your potassium level still in the normal range and nothing abnormal in it. Diet and certain drugs(in your case lisinopril) can cause slightly high potassium. However this won’t cause any problem.
2-Older men may have high PSA, PSA can be high if there is any inflammation or infection. Although PSA is a marker of prostate cancer, your value doesn’t go in to “suspicious” range. As your physician rightly said, we can repeat the test after couple of months and see whether it persists or not.
3-Calcium is still in the normal range! As you age, there is physiological wear and tear of bone and that may contribute to high calcium.
4-Nothing serious to worry.
We can wait few months and repeat the results.
Thanks
