Kindly let me know, is this swelling associated with a local groin swelling? This would help me to rule out inguinal hernia (here the abdominal contents herniate through a defect called inguinal ring into the scrotal sac). You can also share a pic.
This could be due to an infection and inflammation. It may be orchitis, which is primarily an infection of testicles or epididymitis which is primarily an infection of tube lying on the back of testicle where sperms are stored.
This presents as testicular swelling, increased sensitivity and pain.
Another possible cause is hydrocele where there is a collection of fluid in the sheath surrounding the testicles. It also presents as swelling, mild pain, discomfort and dragging sensation.
Kindly visit a surgeon for a complete evaluation and work up. You would need an antibiotic course (usually Doxycycline) for the infection to subside (for about 4 weeks).
For the time being, you can take anti-inflammatory analgesics like Advil for pain. Avoid lifting heavy weights, wear tight-fitting underwears or athletic cups for scrotal support and apply cold packs. You should take bed rest with minimum ambulation to avoid an increase in the swelling.
