Question: My 3 year old sons big toe got smashed with a heavy wooden board. Underneath his toe nail there's a blood clot and its swelling. What should i do?

If there is no active bleeding, just apply ice and leave it alone

Hello and welcome to Ask a Doctor service.

If there is no open wound and no bleeding on the outside, it is best to just apply ice and leave it alone for now.

If there is however bleeding and open wound then you must clean it and apply a tight bandage to the wound so that bleeding stops.

The nail bed wounds generally tend to bleed profusely so the bandage must be tight enough to stop the bleeding.

If you are not sure of how to do it, then you should go the emergency.

It will become painful, which would require a pain killer like ibuprofen.

Based on the weight, you can give 100 milligrams of ibuprofen three times a day for the pain.

I forgot to mention it happened 3 days ago, if that makes a difference. His toe has just been in pain. He can walk on it but it hurts when he does. Theres no liquid coming out of his toe. Ive just been keeping him in bed with his toe elevated.

Since the pain and swelling have started now after 2 days, it indicates that there is an infection.

The collected pus underneath the nail is making it painful and swell up.

Please visit your nearest doctor or PCP and let them take out the pus and start a course of antibiotics.

For the pain, you can give Ibuprofen as advised earlier.
