Question: My 3.5 year old daughter began with severe stomach pain 2 days ago (this is day 3). She complains about a lot of pain for a couple hours and eventually vomits. After vomiting the pain seems less severe for a short time. After vomiting she has extreme fatigue. She doesn't want to move and will just lie on the floor for a while.

Another odd point is that each day has started totally normal with regular behavior, mood and energy. Each day, pain began at almost the exact same time (between 2 and 3 in the afternoon). This eventually leads to vomit and fatigue.

I have gone carefully through your query. Your child has an episode of gastritis which are responsible for both the abdominal pain as well as her throwing up. This leads to fluid and salt depletion leading to fatigue. Proper fluid replenishment with ORS (oral rehydration solution) should be done. Unless this is done, it could lead to fluid and salt depletion. Giving juicy fruits like citrus fruits should help in replenishing the salts and would ease the situation. You also need to give some antiemetic like ondansetron when needed. Give paracetamol or acetaminophen to bring down the temperature, if raised. Also give tepid sponging.

You may give Lanzol Junior 15 mg for the next few days. Please be patient. But if the child grows lethargic you would have to rush him to the hospital. Monitor the urine output. If it is reduced you would need to increase the fluid intake and if that does not suffice you might need to visit the hospital.

She's sleeping right now but spent most the day quite lethargic. She didn't want to do anything but lie down, which as you know is unusual for a 3 year old.

We've been giving her Pedialyte and she's urinating so we thought she's not too dehydrated.

Your sentence about rushing her to the hospital if lethargic concerned me: is she in danger given her behaviour?

Thanks for the update. If she is lethargic it would be advisable to get the CRP (C Reactive protein level) checked and to treat with antibiotics if indicated. Please continue to give Pedialyte.
