A sudden onset fever without any other associated symptoms probably suggests a viral illness. If he is otherwise playful and feeding well, nothing to worry. All that is needed is an antipyretic for 48-72 hours. You can continue tylenol every 6-8 hours for 48-72 hours. Only if fever is not coming down in 1 hour of tylenol, you can give Motrin(Ibuprofen). As per his weight, you can give 3.5ml of Motrin suspension(100mg/5ml), maximum 3 times daily.
If fever is not coming down in 72 hours, he becomes dull, refuses to feed or fresh symptoms gets added up, kindly take him to doctor for further evaluation
