My 8 Months + Daughter Is Having Rubbing/Grating Sound In

Question: My 8 months + daughter is having Rubbing/Grating sound in her knees and we can feel lower bone movement in knee moving sideways since past 3-4 weeks ( we first observed this when she had viral Fever 3-4 week back other there was no such sound earlier).in Normal course she is playing moving legs , started crawling, started Started to standup by her own by taking support .

We visited a Pediatrician Ortho and he first suspected that it's patella of knee that could cause however later when he observed the knee ( we irritated our daughter) he updated that this is not related to patella and could be some ligament( he requested X-rays )
He also said baby has dimples on both knee and patella seems to be small.

He ordered X-ray of Pelvis and knee.

On seeing X-rays he told that baby has a mild hip dsyplacia on the left side however he said nothing is required for the moment and we will followup with after 2 months and lets see the baby development. On Manual Examination earlier he cleared that there was no problem in Hips.

He told even if it remains then the kid needs to be sit in a harness to correct it and there can be no intervention till 3-4 years.

1) Can you please confirm that the dimples in the knee are normal for this age.

2) What could be possible cause of this grating/rubbing sound in both knees and movement of bones sideways.

3) Could this grating/rubbing could cause pain because out of 10 instances for example we see in 1-2 instances she is already irritated and shout and then when we take her in our lap or give her something to play she seems to be having no issue.

Detailed Answer:
Hi XXXXXXX I am Dr. Gopal Goel Orthopedic Surgeon.
I have gone through the details. It is very difficult to comment on some other doctor's opinion without having any first-hand clinical picture. I will try to answer your query pointwise.
1. Presence or absence of dimples will depend on chubbiness of child- If the child is healthy and chubby- dimples may be present but in a thin child, no dimples will be visible.
2.A common cause of grating and sideways movements could be a possible swelling in the knee joint which is a reaction to any febrile illness. It may not be visible from outside but can be diagnosed clinically and on MRI. It usually takes 3-4 weeks to disappear.
3. The swelling could be painful -yes. That may be the reason for her irritation and shouts. Child forgets the pain when is plying unless the pain is severe.
I have tried to answer your questions. Please feel free to ask any other question which comes to your mind.
Thanks.

hI,

1) When we discussed with doctor that was the sound and moment is related to viral infection he said it's not and a co-incidence.

2) like i mentioned in 10-15 instances he is shouting in 1 -2 cases ( not sure whether it's due to pain or general irritation because after that in most of the case she takes feed and sleep).

3) Shall any medicine be given or what course of actions you suggest we should take further to find the root cause.
