Question: My MRI indicates I have a 5mm T2hyperintense focus noted posterior and inferior to the right facet joint likely a synovial cyst. also bilateral facet joint arthrosis at L5-S1. There is also flattening of the thecal sac at L5-S1. There is a lobulated disc protrusion 3-4mm and an annular fissure is noted at L4-L5. Any of these things causing my pain?

I have gone through your query and here is my opinion.

As per your MRI Report,you are suffering from a protruded intravertebral disc(PIVD),called as "Slip disc" in layman's language.
Due to disc protrusion,there is decreased lumen of foramina of spinal canal thus causing a pressure on the nerve roots originating from the spinal cord.
This causes neuropathic pain through out the distribution of the nerve.
There would be severe low back pain and pain radiating to the lower limb along with the nerve.
Synovial cysts are benign and incidental findings which hardly have any clinical implications.

Treatment protocol of such patients include pain killers,muscle relaxants,antidepressants,loading doses of Vitamin B12(Methyl cobalamine).
Mainstay of treatment is physiotherapy,life style modifications,use of hard bed and hot fomenting.

On the L3-4 level there is also a 2-3 mm broad disc protrusion which results in the flattening of the thecal sac and there is forminal exit zone compromise noted. The L5-S1 has a 3-4 mm central disc protrusion resulting in flattening of the thecal sac. Anterolisthesis is noted of L4 over L5. My job requires a lot of physical activities including wearing an equipment belt which is 25lbs. I am having a hard time working with the pain. Do these findings on the MRI support the back pain I experience?

Yes...for sure,it does.
There is a considerable disc protrusion at two levels with foramina exit compromise and resultant nerve compression.
This is the cause of your back pain and it might get augmented due to your physical exertion,wrong posturing and lifting heavy weights.
Please see a neurologist for further evaluation and managements.
