My Breast Surgeon Who Has Mammogram And Singram Techs In
Question: My breast surgeon who has mammogram and singram techs in his office found in my yearly check a 6 mm by 6 mm round mass. He did a core biopsy with a marker yesterday Tuesdsy. He told me that the results will not come in until Friday or following Monday. Okay that is fine. But, next day after biopsy, meaning today the office call to say my results are in and make an appt for tomorrow, Thursday or Monday. No office hours on Friday. Can a pathogist do this in 24 hours all the testing. Obviously for the Doctor to receive the results fast it must be positive for cancer!
Brief Answer:
yes possible in 24 hrs
Detailed Answer:
Hi
Thanks for your query. I understand your concerns.
It is quite possible to do small biopsies like this in 24 hours. They usually do the just the morphology (appearance under the microscope) and no special tests. Special tests are done only if cancer is confirmed and those take 2-3 more days time. These may be ordered directly or after consultation with the patient.
So there is no way we can say that the result is positive that's why you have been called back early. It can be the other way around too. You really have to wait till tomorrow to know the result. Hopefully it will be good only.
Hope this helps.

If I understood correctly, the pathologist just did the morphology appearance under the microscope and didn’t do the special tests due to it might have not found cancer or cancer was not confirmed?
Can they tell the grade and type of tumor from a morphology appearance under a microscope? I guess I am answering in 24 hours
Brief Answer:
Yes grade and type can be told without special tests
Detailed Answer:
Special tests are usually done after surgeon requests for them after getting the preliminary report. Exact protocol varies between centers and physicians.

So basically the report can have positive for cancer, type and grade. Why do special tests and what is it done for since it was a core biopsy. Ti me this mean cancer since it was given soo fast to the doctor.
Brief Answer:
Still it may not be cancer
Detailed Answer:
Special tests are done for estrogen, progesterone and her2 receptors. These are not essential at the initial diagnosis of cancer and hence may not be done at the first go.
Just because it was sent fast doesn't mean it is cancer. It can't be taken for granted.

To update I was correct. The pathologist sent a quick preliminary that said it is duct invasive breast cancer – grade one even though it is 6 mm by 6 mm on sonogram. The pathologist is still doing the special testing’s and it will take a few days. In meantime I’m having a breast MRI and surgery is scheduled for April 3. We Do not know if it even got to the lymph nodes and that will also be tested I’m assuming. The tumor will be sent to California to a new place for staging, how aggressive it is so wanted so forth for future treatments.
My question is if I have a grade one and the surgeon is speaking about after removing the tumor to see how aggressive it is, didn’t the pathologist already tell you how aggressive it is by grading it a 1?

Brief Answer:
Yes it's low grade
Detailed Answer:
The final grading and size is always after the surgery, plus they look for other high risk features on the tumor or any involvement of nodes..ER, PR and her2 receptors testing also required.
However, as it's tiny, outcome of treatment is excellent.
