Question: My brother-in-law had something happen to his brain overnight . . . he went from fully functioning to someone who can't talk, doesn't seem to understand anyone, doesn't know where he is, can't do simple tasks on his own and has very little indication of communication with a lot of repetitive "I think, I think, I think, . . . " the doctors had to release him due to overcrowding due to COVID and my sister-in-law doesn't know what to do.

Brief Answer:
A correct diagnosis is necessary first.

Detailed Answer:
Hello and welcome to "Ask a Doctor " service.

I have read your query.

Seems like an insult to the brain has happened in your brother's in law case.

Some substance use may cause acute confusion too.

It may be an ischemic stroke, an hemorrhagic stroke, or a seizure.

In some cases, ischemic stroke may be transient, so, in such conditions, it may be treated at home.

Before to decide if it's something that can be treated at home, a complete evaluation with brain imaging ( MRI ), cardiac work up and blood tests are necessary for a correct diagnosis and treatment.

If the condition is thought to be treated at home after these evaluations, you should follow the Doctor's recommendations.

Hope you found the answer helpful.

Let me know if I can assist you further.
