Detailed Answer:
Hello and welcome to ask a doctor service. I have reviewed your query and viewed the image.
In this image I can see a dry, red, scaly irregular plaque. It seems like eczematous dermatitis, possibly psoriasis. Does he have this anywhere else? scalp? arms? trunk?
And does it itch? I request you to kindly upload a few more images of the affected areas in " Reports Section"
Take care. Hope to assist you further with this.

Brief Answer:
Possibility of deep fungal infection
Detailed Answer:
Thank you.
I would keep a possibility of a deep/ subcutaneous fungal infection- given the fact that it is localized and unilateral and after considering the duration, slow progression and after looking at the morphology- which suggests a chronic, granulonatous lesion. I would suggest a skin biopsy and staining of tissue with stains for fungus as well as tissue culture for fungus to confirm the diagnosis. If that is indeed the case, he may require systemic antifungal agents.
Take care.
