Question: My child is 7 years old daughter . She has enlargee adenoid . Last year adenoid size increased and nose were completely blocked . But by antibiotic, lanolin inhaler God' s grace she was cured.
From 2019 August she is suffering from skin rashes all over the body sand-like , ringworm . In September she was cured. But again from Nov 14 the she again has ringworm type of rashes on neck, chest and suddenly she caught cold cough, nasal blockage ( suspected due to adenoid) and after 2 days nasal passage has opened by giving Futicasone nasal spray and amoxicillin 400mg 5ml twice but severe cough persisting there with nasal discharge . But the main problem is " nasal sound during speexh" which is 1st time.
When will the nasal sound ne normal?
Whether any other antibiotic to give?

Hii. Your daughter is having enlarged adenoid. Adenoid is generally enlarged in child and it decreases with age. But as your child is having continuous cold and cough then you should consult an ent surgeon and get a nasal endoscopy done. See what is the grading of adenoid enlargment. You can do a x ray soft tissue neck also to see the grading of adenoid enlargment. If it is grade II and more then you should start mometasone nasal spray for long period. The infection from adenoid is the source for allergic cough. She can take steam inhalation after spray. Sometimes adenoid affect hearing.If she is having hearing loss, ear pain also then do a hearing test and impedance audiogram. If she is having snoring, mouth breathing, difficulty in taking food, choking spells at night, abnormal dentition and difficulty in speech then she need surgery and removal of adenoids. But surgery is only indicated if she is having above symptoms. Augmentin is okk for her. No need for other antibiotics. She can take antiallergic tablet levocetrizine for short period. Follow the above guidelines and review with reports or you have any doubt.

her weight is 27.6/28 kg. she is given 400mg/5ml syrup 2times 5 ml for 5 days. But previously doc at XXXXXXX gave 3 times a day. Is it required to increases the dose or to shift to azithromycin as her cough is frequent enough sometimes.

Hii. Augmentin is perfect. You start mometasone nasal spray and solspre saline nasal spray. This will decrease the allergy. Start betadine gargle also and warm saline gargle. Take steam inhalation. You can give augmentin tablet or augmentin syrup around 40 mg /kg. If the symptoms are severe then child can take 80/kg twice daily. Do a x ray soft tissue lateral view and nasal endoscopy.
