Question: My daughter 6.5 yrs old weight 24 kg was suffering from both cold and urine infection.
Levofloxacin 125 mg course for 5 days have been completed but she has got cold and it has complicated.
Now fever is coming at 4 he interval. Her both NOSE is BLOCKED. tried nasivision, Otrivin, metaspray but her noses are blocked and she is breathing by mouth. She has YELlow discharge from nose.
What antibiotic to give???

I have gone carefully through the details and understand your concern. Let me tell you that most often cough and respiratory infection is due to viral cause. Irrespective of treatment they usually last for about a week or so. If it was to respond to antibiotics, Levofloxacin would have sufficed. Management is supportive and antibiotics are not likely to help. You can give some antiallergic like cetirizine, and also saline nasal drops in each nostril to relieve the blocked nose. The yellow discharge from nose does not indicate a bacterial infection. It is due to the dead cells from the inner lining from the nose. You may apply Otrivin but do not use it for more than a week. It makes the child more vulnerable to catching cold.

You need to give steam inhalation to your child. It helps in reducing the congestion and thereby the cough. Just sit with the child on your lap and place the source of steam before you. The water vapor will be in the air and automatically the child will breathe it in. Water vapor will reach deep inside and would help in diluting the cough and thus would provide relief.

If your child is otherwise active and playful, I would not be concerned. If not, it would be a good idea to visit a doctor and to get the child evaluated. If the respiratory rate is greater than or equal to 30 breaths/min you would need to take her to a doctor for the possibility of underlying pneumonia. If the rate is lesser, you need not worry.

Regarding the urinary tract infection, she needs to drink plenty of water. In such a case it is also important to treat with appropriate antibiotics and I would be able to able to comment after you upload the report of urine test.

I hope that I could clear your doubts. Usually the symptoms resolve in a week or so.

I hv given cepodem 50 7.5 no from yesterday night from previous prescription. Her nose discharge colour slightly changes to whitish remaining faint yellow. But fever remaining on higher side .Can I give her clavam stopping cepodem.

Now from 16:00 hrs his fever reduced to 98 for 1/2 hrs.Then again increased remaining around 100.
Don't know what to do?

Ofloxacin seems to be a better choice

The fever seems to be due to the urinary tract infection. Although sensitivity to Cepodem has not been checked, the similar groups are not so sensitive. Sensitivity to Clavam also has not been checked and therefore cannot comment on it. Giving Ofloxacin seems to be a better idea. You also need to discuss with the treating physician.
