Question: My daughter who is 2.5 years old has got some skin rashes behind her both ears few days back and it is getting worst. Not sure if it is an allergic reaction or something else.

She also had Eczema kind of rashes few months back and paediatric dermatologist suggested to use Fucidin H cream. She also keeps getting small rashes and starts itching and it mostly goes by itself after sometime.

We, her parents also have Eczema problem and get itchy rashes.

Kindly refer the attached image and suggest on what can be done for this.

Hello and welcome to ask a doctor service. I have reviewed your query and viewed the image.

I can see red, dry, scaly skin involvement of the retroauricular skin fold and neighbouring skin. She has Seborrheic dermatitis and involvement of retroauricular skin folds is one of the classic sites. Other sites that too can be affected in seborrheic dermatitis are scalp, eyebrows, eyelashes, naso- labial fold and chin etc I suggest that you use a moderately potent topical steroid on her affected skin areas e.g fluticasone propionate 0.05% cream (Flutivate cream), twice daily for a few days

Please see if you can answer below questions.

1. Should we remove her ear rings? Can this be a cause for her condition?

2. I remember the earlier who gave Fucidin H also mentioned it is a mild steroid. Can we apply this cream instead of Flutivate cream? Kindly note that Fucidin-H is given for rashes in her legs and not for her ear condition.

3. How many times in a day we should be apply the cream and for how long?

4. Is there any side effect in using these creams as she is just 2.5 year old?

Seborrheic dermatitis

Detailed Answer:
Hi.

This is NOT contact dermatitis due to ear rings as there is skin involvement away from the ear rings also i.e the retroauricular folds. This is primarily Seborrheic Dermatitis. Anyways it is advisable to remove the ear rings as itching and scratching may lead to secondary infection.
Fucidin H cream is very mild steroid. I would rather suggest a slightly more potent steroid e.g either Momate-F cream or Flutivate cream. Use twice daily for a week or 10 days. There is no side effect if these creams are used for the prescribed duration.
