My Father (77 M) Has What Seems Like A Recurring

Question: My father (77 M) has what seems like a recurring UTI after a lithotripsy. He just finished a second round of antibiotics, but his main symptom mild delirium has returned. We gave an at home test which showed a trace of leukocytes but negative for nitrites. I’m worried it’s a misdiagnosis for something else. I’m not sure what to do?

I can certainly understand your concern

I have worked through your attached query in detail with regards to your father.

Symptoms of delirium include altered sensorium, confusion, and disorientation to time, place and person.

These symptoms even though mild should require immediate hospitalization.

Traces of leucocytes is not an alarming reaction, although one should be cautious of return of urinary tract infection.

Delirium is quite common in elderly people especially those who have recently received medical or surgical procedures.

He’s been to the urologist twice with the same symptoms and hospitalization was not necessary. Could this be caused by something other than a UTI like Interstitial Cystitis? What causes delirium?

Delirium has wide variety of causes.

Delirium require diagnostic confirmation before further action like hospitalization is required.

Infection can be one of the common causes of delirium in his case.

Interstitial cystitis can be a reasonable possibility.

Inadequate treatment of infection or electrolyte disturbances can precipitate delirium.

Delirium should have high index of suspicion and hospitalization is always a safe option for early observation.
