Brief Answer:
The Dytor dose should be increased with spironolactone

Detailed Answer:
Hello,

Firstly, Dytor and lasix are same type of medicines, but Dytor has less side effect of causing low potassium. So it's almost always a preferred medicine for chronic usage.

Here, since he is having swelling, which means he needs a higher dose of Dytor. So I think, he should have 20 mg in the morning and 10 mg at 4 pm as well. Next thing, along with both Dytor and lasix, we usually give medicine called as spironolactone, which increases the potassium level and nullifies the effect of Lasix or Dytor. This spironolactone can be given if creatinine is less than 1.8, so check his creatinine levels in kidney reports.

Anyway, lasix can be taken with spironolactone so as to reduce the chances of low potassium. Also, spironolactone prevents a reduction in heart function so it's very much advisable.S discuss with local Doctor and get it started.

Next thing is low potassium is very dangerous for patients with low heart function so it should be checked frequently till it stabilizes. Also, he should have low water and other fluid intakes like milk, tea etc. You may give him potassium-rich foods like banana, fruits, coconut water 50-100 ml day, but his potassium on weekly basis till stabilization.

Hope this helps you and get back if you have any doubts.

Regards

Brief Answer:
Various options for constipation are Cremaffin plus, Lactulose Syrup

Detailed Answer:
Hi,

You may discuss with the local doctor regarding alternative options for constipation like Cremaffin plus or Syr Lactulose or Polyethylene glycol, which may be helpful in his case.

Also, for sleep we usually give Zolfresh 5 mg to our cardiac patients, so request your doctor for the same.

All the best.

Brief Answer:
Its bit higher, but should see 24 hour output

Detailed Answer:
Okay his output is on higher side, but acceptable. We should see his 24 hour output and intake. Intake should include water, milk, tea, coffee, fruit juices, dal water etc, that is all liquids, not just water.
Day output may be higher due to morning dose, and then night output may be less to compensate it. So our target is intake equal to or slightly lower than output. So if his output is excessively more than intake in 24 hours then we need to decrease the dose of Dytor.
