He seems to have Diabetic Nephropathy.

Hello
Thanks for your query. Based on your statement and reports of your father that you have uploaded which revealed 3+ sugar in urine and raised creatinine with lowered GFR strongly suggest him to have what is called as Diabetic Nephropathy .

You need to do few more following investigations
1) Ultrasound Scanning of abdomen.
2) Serum Electrolytes
3) Serum HbA1C
4) Blood Sugar levels

Consult qualified Nephrologist for further management.

Since you have not mentioned about his symptoms I can not make any comments as regard any possibility of having concomitant Benign Enlargement of Prostate which is common at this age .
