My Father Is Having Difficulty In Passing Urine Since Last

Question: My father is having difficulty in passing urine since last 6 months. Sometimes he has noted blood in urine as well. He has pain in stomach on various occassions.We have done his sonography. one doctor is saying to do operation for kidney stones with laser and to remove prostate gland. Please guide on following

1. Should we go for operation?
2. Which is best method for renal stone opearation?
3. Which is latest technique for prostate surgery?

Money is not constraint for me but I want to ensure comfort of my father. Also please suggest some good urologist in XXXXXXX as we are new to town

Thanks for query ,

Based on the facts that you have posted your 55 years old father who has difficulty in passing urine and hematuria is diagnosed to have Kidney stone and Enlarged prostate .

I shall be happy if you can post me more details to help me in making precise comments
1) Have you done blood test for Serum PSA/
2) Have you consulted Urologist who has carried out Digital Rectal Examination ?
3) What is the size of the Prostate detected on Ultrasound Scanning ?
4) What is the Post Void Residual (PVR)urine detected on Scanning ?
5) What is the size and location of stone ?
6) What is the status of his Renal Function as determined by blood tests Renal Function Test.

The modality of treatment for Kidney stone and Enlarged Prostate is decided by many factors based on results of these tests.

Broadly best modality of treatment for kidney stone is a key hole surgery what is called as Per Cutaneous Nephro Lithotomy (PCNL) however recently awith new flexible ureteroscope a procedure called as Retrograde Uretero Renal Surgery is being practiced depending upon the site and size of stone .

As regards surgery for Prostate Trans Urethrall Resection of Prostate (TURP)is a gold standard method practiced world wide with variations in energy used to resect the prostate which depends upon the size of the enlarged Prostate and other co morbid factors like age of the patient,cardiac function,renal function ,pulmonary function ,ejection factors etc .

as per my knowledge Dr XXXXXXX Desai ( from Nadiad),Dr Shaha ,and Dr XXXXXXX Desai are well known Urologist in Ahmadabad .

Thanks Sir for reply. Following are details required

1) Have you done blood test for Serum PSA/ -- It is 4.3 sir
2) Have you consulted Urologist who has carried out Digital Rectal Examination ? -- I consulted urologist but not aware about Digital Rectal Examination. He took my dad to other room and mentioned that exammination isnormal
3) What is the size of the Prostate detected on Ultrasound Scanning ?- 29cc
4) What is the Post Void Residual (PVR)urine detected on Scanning ? - No idea sir
5) What is the size and location of stone ? -- I do not know but mentioned in sonography report sir. It is in medical terminology so unable to understand
6) What is the status of his Renal Function as determined by blood tests Renal Function Test.-- Blood and urine test were done last week and were normal.

Request you to see attached report of sonography It can give you some more details

Thanks for feedback and uploding Scan report .

Bsed on the USG findings revealed in report he does not need surgery at this stage .

Since stones in the kidneys are small and non
obstructing can be expected to pass out spontaneously .
As regards Prostate surgery I would suggest to get his Uroflowmetry study done if urine flow is moderate he can be treated with medicine s like combination of Tomsulosin and Dutasteride ( Urimax D )once daily You can take a call for surgery if his obstructive voiding symptoms like difficulty in passing urine ,poor flow,nocturia persist even after taking medicines for 3-4 months.
