Question: My fiancé just died April 13, 2020 and medical examiner told my father in law he had heart attack but why was they blood mixed with looked be mucus on his pillow?

I've checked the photo. Regarding your concern, I would explain that sometimes pulmonary embolism edema occurs during a massive heart attack and this would explain the blood with mucus on his pillow.

Anyway, other possible causes like pneumonia or pulmonary embolism can not be excluded.

An autopsy would be the best tool to clarify the cause of death, but due to COVID Pandemic, this may not be always possible.
