Question: My fiancée (52 years old, female) is feeling weak and dizzy, has bloody diarrhea which started as black and is now brighter red, but diminishing. She was vomiting earlier, with black and red streaks, but that has subsided. She has not vomited in at least 6-8 hours but still complains of nausea. I am treating for dehydration first. She has no dilation of the pupils, and pulse rate is normal, so no shock. Should I get her to the ER?

Have seen your details and I appreciate your concern.
Most of the time bloody diarrhoea is bacterial in nature and usually does not limits itself unless antibiotics are given.
As per shifting to ER is concerned if pulse of patient is in range of 60-110/min and blood pressure upper(systolic) is more then 90 mm of hg and patient is not vomiting and there are no loose motions then just keep rehydrating the patient.Better to give Oral rehydration solution/Electral to patient.As vomiting and loose motions causes severe lose of electrolytes.Weekness and dizziness is due to lose of electrolytes like sodium potsassium and chloride.
For nausea give her oral over the counter medication zofran 4mg.However even after this if nausea and vomiting persists then take her to Emergency room.Get her intravenous shots of antibiotic and anti protozoal along with Ringers lactate and DNS resuscitation.
