My Friend Had Surgery To Repair Three Aneurysms Two Weeks

Question: My friend had surgery to repair three aneurysms two weeks ago. Two could be repaired by going in through the groin but the third one required surgery by operating on the brain itself. She was released from the hospital a few days ago. She has a little weakness on the left side but everything else appears normal. Twice now she has fallen. She was standing at her kitchen counter when her legs just gave out and she fell to the floor. Then two days later it happened again. She doesn’t lose consciousness but she can’t getup right away. She said it feels like her legs just fold beneath her. Once she sits down, she feels ok.. she doesn’t have any other kind of weakness. She called her doctor after the first episode and he said it could be related to her seizure medicine she’s taking as a precaution. She has not had a seizure ever. The only other medicine she’s on is prednisone for rheumatoid arthritis. She was taken of off her heart medicine before surgery and has not been put back on it yet. She’s had no chest pain and her blood pressure was 128/60 when she took it after her fall. I’m worried about her falling spells and wanted to ask if it is a sign of some kind of brain damage? Do you think it’s a temporary reaction while her brain is healing? Thank you for any information you can give me.

Brief Answer:
Most likely transient and related to healing process!

Detailed Answer:
Hi and thanks so much for this query.

So sorry to hear about what your friend experienced and the repeated falls. Both in upright position raise a genuine concern about possible orthostatic hypotension. This occurs when the brain receives less blood as a result of a drop of blood pressure in the upright position. This generally resolves with sitting or lying down. Often, this is preceded by dizziness, lightheadedness, racing heart, etc. This could also be a side effect of her new medications.

Most important things at this time is to avoid heights and upright position without supervision as she can easily fall and consequences may be devastating such as fractures and head injuries. Also, a thorough exam to rule out orthostatic hypotension is very much needed. Also, she should be evaluated by physical therapist for balance as well.

In all, fall precautions should be heightened at this time. Evaluation to rule out orthostatic evaluation. Physical therapy for balance and fall precautions. Kindly provide the name of her medication to me for review and more insights as well.

I wish you both well. Feel free to follow up with me as appropriate and ask for clarifications are need be.
