My Friend's Mother In Law , Aged 73 Years Is

Question: My friend's mother in law , aged 73 years is not well since the last 4-5 days. She was having fever for 4 days ( maximum 101.2 deg F) and this was happening once a day. She took paracetamol tablets for 3 days on the advice pf local doctor.
Today there is no fever but she is very weak. Pressure is also low (117/60). She has almost no appetite and is eating very less. Combined with this she is also suffering from short term memory loss.
Yesterday a doctor was consulted and he has advised Azithromycin 500 which she is taking from today. There is no fever today. Bowels are also not clear. She has a stent fitted. Also suffering from insomnia.

Please suggest how to proceed in this case.

Hello.

As per the history provided by you, I recommend the following:

Considering her age and history of stenting done in the past she belongs to the vulnerable population.

Hence, although she is afebrile now, a thorough clinical evaluation is needed for her. Depending on your geography and history of exposure she needs to be tested for Dengue and also screened for COVID19. Elderly females can also have Urinary Tract Infections which can present in a similar fashion.

Furthermore, following or during an infection there is a high probability of electrolyte imbalance, so that also needs to be ruled out, and this might be the reason for weakness and tiredness. But, most importantly and thorough physical examination is the best tool.

So, I'd recommend that she be examined by a Physician and also tested for CBC, Dengue Serology, Serum Electrolytes, and also Urine Profile.
