My Grandfather Age Of 75 Years Old. He Has Been

My grandfather age of 75 years old. He has been suffering from Alzheimer past 5 years now. I have attached some of the reports about the medications. Recently we did couple of the test that attached. From past day 15 days suddenly, he stopped doing his daily routing like take bath, eating himself not having control in urination, however he is active and can speak and walk. He has difficulty in sleeping in night. He is having very disturbed mind currently. He keeps forgetting some people and few people he still remembers whom he sees frequently. 1 year back I had personally met him and he was very active and looked perfect to me at that point of time but recently I observed lots of reduction in the health condition, he looks weak as well.
My questions:
1.What could be the reason for such kind of health reductions?
2. Is the medication what we are giving is good in off or need to change something?
3.What is best way to keep his memory up & claim?
4. Will he able to do his personal task himself?
5. What is daily route he needs to follow which will help him?
Please help us to get above question’s answer.
Thank you for help.
Alzheimer's disease has been progressing.
Detailed Answer:
Thank you for posting your query.
I am Dr Sudhir Kumar, Neurologist, and I would try my best to help you.
I have noted your grandfather's clinical details.
1. Alzheimer's disease (AD) continues to worsen over time, as it is a degenerative disease of brain.
2. What are the medications he is currently taking for AD?
3. He has to be kept mentally engaged- family members and friends should talk to him, he should watch TV, try to read newspapers, listen to songs, etc
4. It is unlikely. He would need help of other people for daily activities.
5. He should be encouraged to get adequate sleep, eat food properly, and go for regular walks.
I hope my answer helps. Please get back if you have any follow up queries or if you require any additional information.
Wishing you good health,

Detailed Answer:
Thank you for getting back.
I have seen the attachments.
He is on DONEP (Donepezil). I could not find MEMANTINE in the prescription. Could you check if he is taking memantine also? Donepezil and memantine are the FDA-approved drugs for treating Alzheimer's disease.
Has he done specific tests for Alzheimer's disease diagnosis?
1. PET-CT scan of brain
2. Neuro-psychological testing.
